Issues with insulin change

My husband, 35yrs old, has been type 1 since he was 5. His bloods have always been on point, until his last one where his blood glucose was reading at the rate of a non-diabetic. So the diabetes team wanted to change his insulin over from humalog mix 25 to Novorapid and Toujeo. He also only recently been given a libre 2 device (he loves it, despite its faults) He has adhd so information doesnt process like any standard person, so i attend his appointment to show suppport as a member of his team, make notes and be someone who can assist when he needs it. He knows his body and his diabetes and how insulin and food affects him. He has had a rough few months with his mental health and developed a serious frozen shoulder and nerve pain, which is being treated by specialist and these has affected his eating habits, which we believe is why his bloods might have been off. He had adjusted his humalog dosages and with his libre to monitor was in the 5 rate on his blood glucose throughout the day. His diabetic nurse has cancelled his humalog and told him to take 20mmol of Toujeo in the morning and then 3mmol of Novorapid with any meal, not snacks. No more information or explanation.

Two days into this new insulin regime and he has been constantly high (16+) He has hardly had anything to eat or drink because he just goes even higher and he has been having additional Novorapid to combat the high but then a few hours later he is back to being high again. Eating and doing the 3mmol he goes uneven higher. Is it normal when changing over to have an adjustment period like this and should we give it time? He hates being high, he said he feels disgusting, irritated and uncomfortable and this all reminds him of how he felt when he first got diagnosed. He has an educational lesson in a few weeks and i personal feel he should of had this prior to moving across and i expressed this and it fell on deaf ears. He said he wants back on the humalog. Will the diabetes team do this for him?

Welcome to the forums. Blood glucose going high is usually a result of not enough rapid-acting insulin injected for the amount of carbs eaten. I have changed insulin but the new one worked the same as the old one. Speak to the medics because they may increase the insulin dose. If I were you, I would learn as much as possible about food carb content and carb counting. I would also calculate how many carbs he is eating with each meal and snack and I would show this info to the medics along with his blood glucose readings so they could determine how much to increase the Novarapid.
 
Obviously take advice from medical professionals, but I would be asking why an earth someone with years of experience couldn’t have had a quick carb counting lesson and been trusted to get on with it. It sounds pretty obvious that the NovoRapid is not enough for what he is eating. Within a couple of hours of diagnosis 9 years ago I was carb counting and injecting accordingly. What an absolute bind for him to be feeling rough.
 
If I remember correctly, toujeo take 3to4 hours to work properly. Try taking at night before bed if it make a difference inform your D/N that you switched. By the way I take 20 units at bed but l take Apidra as and when needed
