So going on holiday to Sicily and panicking about the food...
It doesn't help that my favourite things are cannoli (e.g. Godfather: leave the gun and take the cannoli) and arancini!
 
Meats and cheeses. Tomatoes. And stuff.

Basically have the antipasto without the main pasta course.
 
I have been on holiday in Sicily before and after diagnosis. Low carb food can be a challenge at times and requires some effort but I wouldn't let that put you off going.
Breakfast can be a problem in small hotels, B&Bs etc. as it is usually breads, pastries and cakes, larger hotels have eggs, ham cheese. I like a small snack at lunchtime and I found buying something in a supermarket was the best bet. Evening meals are just a case of selecting a suitable restaurant and ordering low carb stuff from the menu. Don't worry about the 1st plate, 2nd plate etc layout of the menu, that is a traditional presentation, just order what you want.
P.S. I always have the house wine which was usually very good and extremely cheap.

I last went to Sicily pre-low carb, but have just been looking in my diary to see what we ate: I had lots of fish, a rabbit casserole is mentioned and quite a bit of steak and kebabs. Very little mention of pasta, even then. As others have said, if you can get to a shop for your lighter meals, the hams, cheeses, vegetables are the stuff of dreams.
