Ketones in pregnancy

Hello,

Bit confused... nothing new there!

I'm 35 weeks pg and just had an appointment with my midwife. My urine sample (fasting) showed ++2 ketones, and some protein and my blood pressure is a little high. My insulin requirements have gone up quite a bit since I last saw her, Lantus just by 2 units a day and Novorapid almost doubled but I felt like I was on top of things by cutting down ever so slightly on my carbs. My sugar levels are good, averaging around 6. Not sure what I can do or where I've gone wrong... :(

It sounds like you are doing amazingly well so far, to be at 35 weeks and not be in hospital full time is good in my book . I was in full time with this ongoing problem from 6 months with my first, even with a HAb1c of 5.8....I just think it is all the extra demands the baby puts on you towards the end of the pregnancy. With my first my Endo said I was on one of the highest doses of insulin he had seen in pregnancy towards the end of my third trimester and I am usually so insulin sensitive and on tiny doses. I had Ketones and protein also but from a lot earlier on than you.
I don't think you have gone wrong at all and if your average BG is 6 you are doing all you can, and by attending all your ante natal appointments you can do no more really . Nature will take its course and if at all you start to feel very unwell,like an excruciating head ache, a very swollen face and hands and shortness of breath then obviously you would need to get yourself straight over to A&E as this could be pre-eclampsia....but it sounds like you are being monitored well so try not to worry too much I know it is not easy but not long to go now and you will have a lovely new baby

Sorry you had a difficult time with your pregnancies, how old are your little ones now?

I'll be keeping an eye on my ketones and my midwife says if I start to feel unwell then to call her straight away, and I'll see her again next Friday anyway so yes, I'm well looked after!

Do you think it's a good idea to cut back on carbs slightly at this stage... my diet is normally quite balanced but now I don't know what to eat for the best!!

There is no way someone on a forum should advise you on your diet at 35 weeks gestation, it is too complex and I don't think you should be trying to reduce food groups at all this far into a pregnancy.
What did your Ob say? Your Endo?
I think you should ring them with this question at your first chance and take their advice and don't panic into doing anything .If at all you feel unwell as they said don't hesitate to go in.

It does sound like you are doing great and I remember a Midwife saying to me that all pregnant ladies have a touch of Protein and some Ketones towards the end due to the weight gain demands of the baby, Diabetic or not. Just try and chill and don't forget to ring and ask their advice.

I think I tend to panic when I don't really understand something! My ketones seem to have dropped so I'm a bit less worried now, hopefully all will be fine at my appointment on Friday.
