Lantus Hypo

Hi guys,
It's been two weeks since i started using lantus, and everyday am having 100 point drop from sleeping test point
so for the last week I start sleeping with 180 so I wake up in 80-70 range, is that ok or going to cause me an issue after along time?

I tried to reduce the lantus dose from 15 to 12 and nothing changed.. for the first week I was injected on 10:30pm, now I change to 8pm.. nothing changed, i was told that lantus does not have a peak that reduce bg by 100 point..

I am thinking that the pen is not working properly.. is it ok if I put my pen at the door of the refrigerator? what about if the pen was on 35C for like 30min.. does that ruin it?

last thing, does lantus put the BG at certain level based on the dose or it just maintain the current level of my bg even if it is 300?

At 35C I highly doubt it is okay anymore, I store my insulin in the door shelf of the refrigerator. Generally, Lantus should drop you by no more than 40 points, mine drops me around 80 points but I am happy with it. I don't find it a big issue "personally" to sleep at 180 mg/dL and wake up at 70-80 mg/dL range but that is just me. We are all different so it is a bit of trial and error, let's see what others have to say. One last note, have you tried testing after 2 hours of injecting Lantus? I found out that Lantus drops me after about 4 hours so after 2 hours, if there is a drop, it is either caused by Lantus (being spoiled from 35C, high dose) or by remaining bolus from last meal if any.

Regardless of dose, Lantus will last up to 20 hours as maximum. For me it is 19 hours. I think you should reduce 1 unit and see how it goes if that drop is causing you all the trouble. Consider it.

How are you dealing with your BG after that 19hours? did you take any snack with novorapid to cover up the rising?

Why not try splitting the dose by taking less at night and more during the day?

Typically, the Lantus lasts between 20 and 26 hours at full strength.
For me this was about 24 hours.

However, I believe low levels of Lantus remain for up to 4 days. I noticed this when I went from Lantus to pump.

Therefore, when changing the dose, it will need at least 2 days to "bed in"

But remember, we are all different. Only you know how each insulin works for you.

Another thing to consider is a basal test. Long acting insulin should keep you at a steady bg. If you can fast for 8 hours at a time, you can observe what happens to your bg when it has no carbs yo raise it or bolus insulin to drop it.

I think you mean from Lantus to pump

do you mean spilt it 50/50? or 70/30?
so will I face any overlap between the two doses?

Unfortunately that's only something you can find out through trial and error, but id probably start by taking a bit less at night since you are having hypos and more during the day. Then adjust based on your results . Ideally some overlap is okay and you want it to be in the morning to help tackle dawn phenomenon, but everyone is different .
