Lantus Splits

Hi all,

I have been reading Bernstein's book and would like to know from others that have tried it before what the effect is on your blood sugars when you split your daily Lantus (long lasting insulin) dose from one injection to two.

Did anyone experience lower sugars, higher sugars etc? I would presume smaller doses that my body would use more effectively would mean that my sugars are likely to drop.

If anyone can advise me on their findings before I try, it would be greatly appreciated.

FYI - I currently take 20 units of Lantus around 7pm each night.

Hi Sarah,

Mine works effectively at the moment so if there is no benefit to be had from splitting it then I will leave it as is for now.

Im not due to see them again until Jan (once yearly) so I will ask the question then and see what their response is.

sarah is totally right,if its fine dont change it,i take my lantus every morning and that works fine so wouldnt change mine unless things obviously changed :D

6.3 before lunch

5.3 Mid Afternoon

Now 7.3

Would you suggest I need to change Lantus as it has increased slightly an hour before next one (as per Sarah's post)?
 
Hello Paul,

As is usual, Mr Bernstein is spot on in his observations about insulin dosage, absorption and predictive action. Splitting your Lantus dose not only assures true 24 hour coverage, it will
give a more even and reliable action.(no more hypo days/high as a kite days)

Most people start by splitting 50%/50% AM/PM. I noticed an immediate improvement in control
with this split, but (after a bit of fine tuning) ended up with a 60%/40% split. The only price
you pay for the better control is an extra daily injection.

Another option to splitting is changing the time when you take insulin... Either pushing it forward a hour or back a hour might help....

I found that with Levimer that tends to lends it self to splitter better than Lantus... That not only did I have to split but also adjust the timing to get the best effect...

It was a pain as I ended up with a 60/40 split at 11.30am and 11pm, but with saying that hubby takes his levimer in one shot at 10pm no probs
 
Thanks everyone for all the info.

I am getting the sugars in the right range now and as you all say it looks like a bit of fine tuning should do the trick.

If I split 20 units to 10 & 10 im worried that I might drop low as theoretically my body should make better use of the insulin.

Timo, did you find this when you first split or were things pretty settled?
 
I decided to split my Levemir morning and evening. I suggested it to my diabetic nurse who said 'why not'. My overall intake is exactly the same, - 28 - but split 10 am and 18 pm. Seems to work fine...I don't know anything about Bernstein (who frankly sounds like hard work) but it just seemed entirely logical to me...
 
Hi Paul,

Things were fairly settled from the start, although you need to give it a couple of days
to even out properly. I found that I needed slightly less insulin with my evening meal, due
to the improved basal cover later in the day.

I play footie saturdays so I will wait until next week and give it a go then so it has time to settle before the following weekend.
