Hi,
I'm on Lantus basal and in the process of changing from morning to evening. What is the best time to take it in the evening as normaly it runs out about 2hours before my

morning one at the moment. I normally eat about 5.30 to six pm and don't want to be running high after my meals or before bed.I believe Lantus takes an hour to kick in
Any advice on this would be very helpful as I'm new to this

Hi there, I take mine at around ten-thirty each evening which is at least half an hour before bed & time for a last check before kipping down.

I have also taken to splitting my dose, two thirds at ten-thirty and one third around lunchtime as I felt my blood sugars baseline rose in the evening. This does seem to help, but everyone is different.

You could try working it an hour later for a few days & then another hour until you are happy with it. Remembering to allow your changes the time to take effect is the most important thing I've found.

Hope this helps.

Thanks for that very helpful Is it usual for your bloods to run a bit higher during the change over because I had to take half this morning and half tonight just for today. Then all at night tomorrow.

When I first started on lantus I did take my lantus dose before bed, but in more recent years I've taken it early evening between 6-7pm.

if you have established its not lasting the full 24 hours, then splitting your dose makes sense..........some say Levemir is best suited in these circumstances but Lantus can be used just as successfully..........

it could be argued that your Lantus dose is used up at different speeds throughout those 24 hours, so its definitely worth changing to the evening to see what effect that has on the duration........

I know its most common to take your single dose of Lantus before bed as this means its already fully underway for the morning dump of glucose from the liver and the increased resistance to insulin...

Thanks for all your help I suppose it is just trial and error but it does help if you know what your doing which I don't at the moment. Will just have to try few things as my nurse just said it don't matter when you take your Lantus as long as it is about the same time each day. Only on very low doses at the moment.and 1;30 Humalog I had a very high spike this morning after breakfast Do you think that would be due to the change of timing or something else

It may well be, the manufacturers (Sanofi) say that you should allow up to 3 days for changes to take effect before adjusting doses again, I suppose this would also apply to time changes.

If your unsure how to do basal checks have a read of the following, it's for pump users but he advice about fasting is the same for those on MDI:

I have a look at that and thanks for that. I see my nurse next week so will ask about going on a daphne course but the chances are probably slim as I have only been diagnosed a couple of months.I was just given the basics for a start but I can phone diabetic team any time . I am reading the think like a pancreas book and Type 1 for children and young people by Ragnar Hanas trying to educate myself a bit but a my age 67 its not easy

Two excellent books, I've read Think Like a Pancreas (several times) and have heard some excellent feedback from the Ragnar Hanas book.

The following is an on-line version of the DAFNE course, you do have to register but by all accounts it's well worth it:
