Hi

I proberly should know this already but dont so im asking......before i go moving around the timing of my lantus how do you test to make sure/see howing it works for?

Am on insulin Pump now but when I used Lantus, initally I was taking at night time but after having numerous night time hypos, I then split the dose into 2 and took it in morning and night time

You should have been given an idea by the consultant who diagnosed you as diabetic? When did they say to take it? Is it you who wants to change the times of it? What way are your blood sugars going?

I was told to take it at night, but like you i do wake up low quite often. Ive thought of splitting the dose, how would you go about doing it? Do you literally just split the dose your taking in two?

You need to keep the doses 12 hours apart so I used to take mine (before the pump) at about 7pm and 7am.

My doses weren't split equally - ie one was higher than the other. This was because otherwise I would get hypos at night and I needed more insulin to cover the daytime basal than at night (I think I am remembering this correctly!)

It would be best to ask your DSN or doctor for support on this and also make sure you keep testing to check what happens to your blood sugar and to get the doses correct.

Before trying split-dosing why don't you just move the timing of your injection to earlier in the day, this alone should stop your night hypo's providing you get the dosing right.

When I moved my evening Lantus earlier it was a case of moving it an hour earlier every evening until I got to a time that suited me.
The hospital have always said to me that it doesn't really matter when I take it as long as it is at the same time each day.
I split my dose but they are not 12 hours apart, I take 6 units at 9am and 18 units around 6:45pm.

I'm also considering my lantus glargine injection which I take (18u) at breakfast time. Can you please explain what 'Glucose Infusion Rate' on this graph means?

I was taking 30 units at night but when I split it, I ended up taking 14 units in morning and evening and that worked for me! It took a few days to sort it out though and make sure you get advice from doctors before doing it
Do you eat enough before you sleep? It could just be something simple like that so take everything into account before you split dose! You said you wake up low often? How low? Are you having hypo? How does the rest of the day go? Go check anything that may cause the lows? You did'nt eat before sleeping? Did you exercise before?

Mark from diabetes daily said it was from a comparison trial. I guess it was a glucose load used

I might be wrong but I'm sure when I started splitting my dose it was because my levels were rising quite a bit towards the end of the 24hrs. I used to take it around 10pm yet from around 6pm onwards my levels would go up without any real explantion.
In my case my DSN actually told me to keep taking the same evening dose and add some units in the morning, so although I did split my dose it was actually an extra dose.

I guess you would do an afternoon/early evening basal test till you inject of an evening,
Search here or google for diabetic basal test, to refresh what you were told by your nurse about basal testing. Put the results to your nurse, I guess if it's low overnight and high of an evening she will help you split the dose
