Levemir twice a day..

Hi guys, I’ve been on levemir twice a day now for a few years. I’ve been experiencing strange behaviour in my sugars after my nighttime dose as they seem to be stable but then start going up gradually before bed and then slowly back down when I’m wanting to get to sleep. They have even dropped me low into the night so I’m constantly having to eat more unwanted food at night to prevent them dropping as much! I’ve been getting this sort of thing very frequently without any Bolus insulin (novorapid) in my system.

The only thing I can think it may be is my morning dose overlapping with the evening dose?

Hope you guys can help me out!

What is sounds like to me is that your morning does is too low and you evening does is too high. Try adjusting them and see what works for you. If you think that it is overlapping they recommend taking your evening does at least 12hours before your morning dose. I would also recommend having a chat with your diabetes nurse as they can help you find the right adjustment (if needed) to suit you. Hope you figure it out soon :)
 
Levemir's worked much better for me since I started doing more equal doses 12 hours apart. I'd initially been told "When you get up and when you go to bed, with higher day time and lower nighttime doses." and when I pointed out I often was only asleep for 6 hours or so and not the most consistent of bedtimes just got told to try it. I gave it 18 months, then got fed up with the evening meal sending me sky-high before bed no matter how accurately I'd carb counted and, in my case, hypoing almost every morning before work which is when my overlap seemed strongest.

I've virtually eliminated both since I altered the evening dose to go with my evening meal, which is pretty consistently 12 hours after I get up.
 
Thanks for that! I will try that out without panic eating before bed so I can gage what dose will be right for me

That makes a lot of sense, also my insulin sensitivity seems to be a hell of a lot higher at evening/!nightime and if I’m exercising that can have a lasting effect hours later if I’m right?

What doses did you alter it to and from and what timings?

I have settled for an (approx) 07:00 and 19:00, 50/50 split. But these days my exercise regime is seldom more strenuous than a brisk walk.

Might require a different approach if doing proper exercise.
 
I generally do 6.00 a.m. and 6.00 p.m. give or take 1/2 an hour either way. Same amount morning and evening. It used to be 2/3rds in the morning and 1/3 at night.

If I know i'm doing a lot of physical activity in the day I'll cut back a bit or if I've hypoed a lot. Just now what with walking so little... well it's higher by 2 units. With a note to bring it down once lock down is overm
