Lost my job because of Hypo's

Thanks IHs, not quite sure what you mean about the 2U to 10g my carb ratio's on the pump are set at

05.00 22g
10.00 14g
17.00 16g

Maybe I should adjust these, not sure. Scince 17th Feb I have not had a BG below 14.6 and regularly in the 20's. Its half term, I have had a cold, and am paranoid about using sick day rules correctly in case I crash suddenly. I have so much info written down now to speak to DSN about on Friday. Hopefully I will sort it soon as I have nearly had the pump for a year and although HBA1C has gone from 13.2 to 8.1 then to 9.1 the DSN said they decided not to continue funding as I have not made enough improvement. If I do go back to pens, then I am worried about not being able to deliver such tiny amounts of insulin and having background insulin on board when I start crashing.

sorry for not replying sooner.

I'm not a consultant or dsn but I think if you were to chat with my diabetic team they would probably tell you that you are 'basal heavy' and 'bolus light'. Meaning that although your fasting bg levels might indicate that you need the amount of basal you have entered in the pump, because yr carb ratios are set as they are, your bg levels are not balancing up very well. My consultant is very hesitant for anyone using more than 0.6u/hr and thinks people are better off balancing their bg levels up by using a higher carb ratio. So with this in mind, maybe adjust yr basals down a bit and increase the bolus insulin to carb ratio. The proof in the pudding is what yr bg levels do all day and not so much what your fasting level is. Also remember that most bolus insulins will last for at least 4hrs so it's important that the correction amount is set ok but again that can only be determined by doing bg tests every 2hrs - trial and error.

I'd be inclined to talk to your dsn and ask her to help you set the pump up to work in a better way before she decides that the funding should stop.
