Hi everyone,

I've recently changed from a mixed humulin insulin pen which I was put on when I was first diagnosed to a basal/bolus regime (Lantus and Humalog)because my sugars were so up and down (mostly to high). At first they were amazing and I had such good control but they seemed to have gone back to the beginning again.

I have a weird life style. I have 2 part time jobs, one where I sit in a office on my bum and the other extremely active, on my feet for around 8 hours.

I'm also not doing great with food. I eat different amounts of carbs and sugar depending on how hungry I am. Some points in the day I'm not hungry at all. Other points I eat quite a lot cos I feel really hungry :L

I take 14 lantus in the morning (which I upped from 6 because sugars were high all the time) and then carb count as normal throughout the day but everything is just really up and down.

Any suggestions?
 
Hey @Moparp - can you give us some idea of what your BG readings are throughout the day and what your carb/insulin ratios are like ?

Looking at your post and your lifestyle the basal/bolus regime is best for you, however if either your basal amounts or your carb/insulin ratios aren't right then with some tweaking they could get alot better. Also please remember that ratios can change throughout the day and also lantus might not be the best basal insulin for you, so a better idea of what's happening throughout the day may pinpoint the issue ;)

One of the great things about basal/bolus is the flexibility to eat when you want, so if you get days where your not hungry then that's fine. Your first approach should be to start with some basal testing as without getting your basal dose right you will always be chasing your tail, once you get that right you can then look at your insulin-to-carb ratio's.

For every 10g of carbs I take one unit. I eat different things each meal, each day. I used to eat low carb but that went out of the window.

Today so far 17, 12, 9.9
This is some over the last two weeks. The others ain't been so bad between 5-10 ...

5.4, 12.7, 3.7, 7.5, 3.8, 12.7

6.2, 10.2, 17, 7.7,6.3

5.5, 19.3, 10.5, 11.2, 11.2, 6

12.8, 5.8, 12.2, 14.6,12.6, 8.7, 5.5

Agree with Noble above - basal testing is a good place to start, see if you can start with that ?

Also speak to your team and get yourself booked onto DAFNE, this a great course to understand carb counting but also for general type 1 stuff, will give you some good tools to become an expert at your type 1 management.
