Medical Exemption Certificate

Hello, has anyone had any difficulty with getting a physical medical exemption certificate or dealings with the department and have any luck with getting one? I've been charged and fined the presciption from last year £9.90 + £50 fine. I'm type one diabetic and have been for 18 years. I used to get one sent via post and now they've renewed it but said a different address is on the system (i wrote the application, my surgery receptionist checked it all over and doctor signed it off), i'd like a physical one to avoid getting a charge/fine again in 5 years time when it expires.

I also explained on the letter it states I didn't show my exemption certificate - which was unfair as my chemist knows who I am, regularly gives me diabetic medication so when I picked it up I wasn't asked to provide or even tick the 'E' box and sign! So frustrating.
 
Never had a problem, it just gets sent to my home address every 5 years. Strange that a different address should be showing on the system? I'd certainly be appealing any fine as it's quite clear you are exempt from the charges.
 
I normally apply for a renewal at my GP surgery & it gets sent in the form of a card to my home address.
When I pick up prescriptions my name is checked against the registered home address on the label? (They also ask my address before handing over?)
