Metformin is known for gastrointestinal upsets and issues. Never ever take it on an empty stomach, request the slow release version if he isn't on that already, or try a different method entirely to get blood glucose control. (Metformin makes you a smidge more sensitive to you own insulin and it keeps your liver from dumping too much glucose... It doesn't do much of anything about ingested carbs/sugars, so it's not likely to "solve" diabetes on its own, even in the best of cases.) There are other medications that can help, metformin is just the first one that is tried. And if he wants to go without medication, he could just try a low carb way of eating. It's quite effective. https://josekalsbeek.blogspot.com/2019/11/the-nutritional-thingy.html <-- I wrote that blog a while back. Metformin didn't agree with me either, so I ended up just not eating/drinking the things that elevate blood sugars to a diabetic level. Much more doable than dealing with side effects and complications, methinks, but let him have a read, see whether it might suit.

Good luck!

My GP just insists there is no other medication than Metformin. I am 78 and for many years was on Metformin and PPI’sto the point where I could never leave the house and some days the bathroom. My life was severely impacted. I then started to pay privately to see various consultants who diagnosed possible Chrohns or celiac took me off these medications and sent me home to be managed by GP. HE insisted I go back on Metformin.
Meanwhile 3 months ago I was diagnosed with PMR/GCA and now take long term steroids which are playing havoc with my type 2 diabetes. I am now suffer erratic blood sugar levels and was put on insulin, as prescribed by hospital and is under control all be it erratic in 24 hours. I have now been told by GP I should restart Metformin as well! With the predicted unbearable side effects.
Personally I have had enough and intend to no longer consult my GP. He just will not listen. I cannot change surgery as I live in a rural area and they are the only ones. S my advice would be be aware!
