Multiple insulin dose 2 vs 4

hii everyone i just want to ask what will be better 2 injections of mixtard30 or 4 shots of novorapid and lantus for better mangament......iam recently diagnosed with type 1 diabetes and hba1c were 238 mg/dl.....which is approx 9.9%.
 
Generally speaking mixed insulin regimes are much less flexible - your insulin would have to be based on your basal requirement (insulin active throughout the day to keep BG level), which means you have to get up and eat fixed size breakfast every day at the same time regardless of whether your hungry or not; further, you'll probably have fast acting insulin in addition to the mix (since you cannot correct highs with the mixed insulin) meaning that you will very likely end up with more than just two injections per day.

The "four shots" is called basal/bolus regime because you have to seperate insulins for the basal basal requirement and meals, meaning that you don't have to eat when you're not hungry because the mealtime insulin is a separate pen.
You can make basal/bolus as easy or complicated as you want - you can learn to carb count (basically, a way of figuring out how much insulin you need for unfamiliar meals) or you can stick with fixed size injections for fixed size meals.
 
You'll see the four shot regime called "Multiple Daily Injection" therapy, or MDI as we like to call it.
 
for best control use mdi given those choices. That way, you can eat when you want rather than have to catch the peak of a mixed insulin. Also,dose adjustment for quantity eaten is easier. There is alot of learning to do with either, to get the timing of injections correct, and then adjusting dose depending on meal size, content, activity before and after eating etc. For the very best control the best option is to pump as this gives you the flattest possible basal profile.
 
MDI no question. I was diagnosed in June and put on 2 a day to get me used to everything for a few weeks. I'd never swap back now. Hope that helps! :)
 
Having been on both I would opt for basal/bolus, as already mentioned it gives you so much more flexibility than twice daily doses.
 
I would say MDI as
I started on 1
then went to 2
now on 4 which is much better.
Also the insulin for the purpose so Longer acting for basal and short acting for meals.
 
okk ryt now pumps are not widely available here in india so i think i will ask my physician to prescribe me alternate insulin regime....... thanks fellas it feel so normal to be diabetic among u people ...:)......pls send me how to count carb in indian food.....:)
 
I'm on the MDI regime,which I alter as and when needed,it gives me the freedom to eat or not,eat little or large,and also when. In fact I'm very happy with its versatility,which suits me and my lifestyle.
