Need help with (dla1a) Child Disability Living Allowance form

Hi , My Daughter has Type 1 Diabetes and i have been told to apply for DLA by her Diabetic Nurse. I have received the form dla1a and am in dire need of help. With looking through the forum i have found numerous links to leaflets and sites to help but am still struggling. I also have contacted the local Welfare Rights but have been told there is a 6 month waiting list to be seen.
What i am looking for and feel would be of best help would be a copy of a filled in dla1a form that was granted DLA. I know every child is different in the help they recieve but think it would still be a usefull guide in filling in the more difficult Day to Day help section if i can see how best it is written out as a basis for filling out my childs needs.
I would be gratefull for any help in this as i'm really struggling

Hello. I.ve just done my daughters and we have got it. The dsn wrote a letter to support us. I kept a diary by time of day for 2 weeks where I documented everything I did that I dont do for her brothers e.g checked dose of insulin, supervised novorapid cartridge change, watched injection etc etc also had a section of the diary which was stuff I did after 10pm (my bedtime) as that affects your award. So a few times she awoke with hypos that we tested, treated, retested and gave carbs then resettled her and I put a time on that -about half hour per hypo. Also the extra time it takes sorting out issues at school or wrap around club etc and my monthly faff with the chemist who keep dispensing only part of a supply. Put it all down and if u get a question in the dla form just write refer to diary. Hope that helps
