I have just been diagnosed with hyponatremia (low sodium) blood problems. I have seen a recent study where the use of Metformin can cause this. I have been using Metformin for about 30 years now, and I need to discuss this with my GP. I will add the link to that study if and when I uncover it again. My B12 test is fine, my ferritin is midrange and I am not anemic.

It is a mouse study so treat info with care - it may not apply directly to humans.

If you eat low carbohydrate and drop into ketosis you need to take in extra salt anyway.
Both sodium and potassium, I think.
I would suggest that any diet that leads to significant or quick weight loss will need supplementing especially during the initial period where most of the weight loss will be glucose and water, not lipids.
