New here! Need some advice of people who have diabetes!!

Hello, I'm Laura have been type 1 diabetic for 13 years (24 now) I've never had much of a problem controlling my diabetes until 3 weeks ago it all went stupidly wrong... I ended up in hospital with ketones 5.2 and bloods @ 28.0 - To wrap this up I pretty much nearly died ( due to my lack of knowledge of ketones)
Anyway, on a brighter note... I'm out now, but have lost all confidence in controlling my diabetes and am testing my blood sugars around 20 times a day, mainly because I NEVER want to be in that state again.
Upon coming out of hospital I went to see my gp as I was having the occasional hypo at night, (3 months ago I changed from lantus to levimer due to the apparent 'cancer risk' with lantus) he suggested instead of doing my usual 24units at night, splitting the dose over 12 hours instead of 24hours. I was told to inject 11 units of levimer @ 8am and 11unjts @ 6pm.
When I did my first injection of 11units @ 6pm I thought all my problems would be solved!
How wrong was I.... I woke up the next morning @ 7am to find my blood sugars were 19.6.. For me this was unusually high! As you can imagine I reached for my ketone sticks and tested away.. 0.2. Phew!
I booked in with my gp for an appointment a hour later, by the time i had got to him my blood was reading 23 and my ketones 0.9!! As you can imagine PANIC set in as brought back memories of what happened 3 weeks ago, and under no circumstances want to end up In that state again! he advised me to immediately take my fast acting insulin and eat something, 3 hours later my blood went back to Normal BUT THEN didn't stop coming down, I then had to inject fast acting again and eat, then again another 5 hours later. Again took my levimer @ 6pm thinking this little blip had passed and my body was just getting used to the new regime. But again I woke up the next morning to find my blood sugars were 18.0! I have now realised that the levimer was actually not working at all let alone the new doses giving me better control!
Anyway gp was quite impressed I diagnosed the problem myself (well what else can I do with my time! I've been off work since hospital!) He has now changed me to Humulin, still split doses but now of 14units every 12 hours. This was yesterday, i did Humulin @ 6pm, blood sugars were 5.2 @ 11:15pm (was quite happy with this!) then had a major hypo @ 12:45 (1.9) eat qui a bit if sugary stuff as i just wanted to get rid of the horrible feeling of being low! this morning I woke up with blood sugars @ 16.0 so did the usual.. Inject fast acting.. Eat breakfast @5am may I add! By 11am my bloods were coming down to normal again and not stopping. So yet again injected fast acting and ate, at 2:30pm I had another low of 2.4.. Had another dinner at this point as didn't want to stuff my face with jelly babies again! Then by 7:30 needed to do fast acting again.
WHAT THE HECK IS GOING ON!!??!?
is my body rejecting the long acting insulin?
Am I being a mong and doing something majorly wrong?
Sorry about the rant and bloody life story but I have not had the experience of talking to anyone my age group wi diabetes!

Split dosing doesn't necessarily mean give 50% in morning and 50% at night......it can be much different....
Does your GP look afte all your care? Are you under a hospital consultant?
Sorry, just a quick response.....I know that you are obviously petrified of more high's, ketones and lows....however since I have been on a pump I feel less able to give details on injections...I just know that prior to pump I went to split does and it wsn't on a 50/50 ratio...and splitting the dose did not work for me.....hopefully somebody with more experience will come along soon....and that posting this may bump it up....

Hiya! Thanks for your reply, Yes at the mo it is my doctor that looks after my diabetes only because he is a diabetic specialist aswel as a gp. HoW do you find using the pump now? :D

Hi lillaura87,

I had the same problem after changing from lantus to levemir and then Insulatard, none of them give me the same control as lantus so I eventually changed back to lantus despite having some issues about its safety. I find lantus is a excellent basal insulin and lasts the full 24 hours from injecting, I rarely adjust the dose and don't have site issues as some people experience.

With all respect to your gp but I would ask to be referred to a diabetes clinic at your local hospital, there you can see a diabetes specialist who will look closely at your control and advise accordingly, once you are back in the hospital system for the treatment of your diabetes you will remain there until such times that they feel they can hand you back to the care of your gp.

Should you be fortunate to be referred to the care of your hospital team then you may like to enquire about a place on a DAFNE course (dose adjustment for normal eating) which will cover carb counting, insulin ratio's, basal insulin adjustments and sick day rules as well as other aspects of diabetes management.

My GP was a gp that specialised in diabetes, but he still did not have the knwledge that my consultants do at my hospital. Don't get me wrong he is a great GP, but when he was limiting the supplies of my strips...for a type 1, it did make me think 'how good is he' and I got told by my OHP at work to get the referral to hospital asap as they specialise in diabetes everyday, all day and their knowledge was more...

This was true for me....last year when I had trouble with my driving, thankfully I had registered with the hospital and was under their care...I was put on to a pump within 3 weeks and haven't looked back. I get exceptional care really compared to others, and am really pleased that I am under them.......When I went to pump therapy...my GP who is the diabetes gp for my surgery had never seen a pump...and didn't know anything about them....so does not discuss my diabetes with me at all. They still do my bloods and help me enormously with my fibromyalgia...which because he no longer cares for my diabetes-he has been brilliant with the time he has given to me for my fibro....

Pump....I wish I had it 25 years ago. On my worst days which are very rare now I can go up to 10.....I have had a couple higher either recenty due to a set falure and to eating a sausage roll!!! You do need to be good with monitoring and foods initially, but to me it is worth it.

On MDI I could not stop the rises in my levels in the mornings which always happened an hour after I got up. I came to accept this, because no matter what I did nothing stopped them. Yes, I could take a correction by waking at 4am each morning, or when I got up...but it was a complete pain in the bottom. Now, no more I am at a consistent level and I don't have to worry or bother....
Lows still occur-but are rarer and fixable....i.e living in Wales, we don't often get hot weather...and the first time last year it caught me out, and I am still waiting for any hot weather to come back, but I have a separate basal regime set up ready for it....or I can use a temporary basal......so I have choices to manage myself.......

Eating is so much easier....and more flexible in many ways, but tighter management in other ways....

Personally I wouldn't hand the pump back....the extra work involved initially is more of an effort, and I had fantastic help from somebody here...

If I was your age, (as I was being on MDI) I would not hesitate to swop to a pump.....but you would not get one from your gp....they are totally managed by consultants at hospitals I believe.

Sorry I haven't answered about ketones and insulin, or doing something wrong...just that even after 10 months on the pump, because I couldn't manage splitting my doses and getting good results...I don't really feel from my experience that what I would say would be of any use.....I am sure others will be along that are better than me and have managed it successfully.

I do hope that you are feeling better soon...

:?: Hi Nigel, I had no problems what so ever with lantus, I seamed to be so controlled and could eat when I wanted with lantus. I think if this Humulin doesn't work I'll just have to go back on lantus!

@donnellysdogs! The pump sounds amazing, I've heard lots of good reviews about them :D Im waking up at 4:30 since my bloods are high just because I don't want them to get stupidly high and me panic when I usually wake up (7:30) this diabetes is a pain in the bum at the mo lol!

And I think I may make an appointment with the diabetes team at th hospital! Just to be on the safe side! :mrgreen:
