new insulin

hi,
I was taking glizaide and insulin at bedtime to control my diabetes my readings were very high so was changed onto insulin twice a day insuman comb 25 i am on now morning and evening i was told to take it twice a day half a hour before food,i was told to take 10 units and to increase it if my readings were high.
The same week i was admitted to hospital for a pacemaker fit urgently on fitting the pacemaker they punctured my lung so was kept in for a week,in hospital they said it did not matter when i took the insulin as long as i took it twice a day,i still tried to take it half a hour before food,i have kept increasing my dosage as my readings 14 in a morning and more often than not 20 at bedtime i am now taking 46 units twice a day what am i doing wrong surely my readings should be lower than this.
I would have thought they would have dropped drastically my appetite has gone my weight has dropped 18lbs in a month i know i can stand the weight loss so not worried about that just not sure whether this is due to change of insulin or the op.
I have made a appointment with my diabetic nurse but its in two weeks just wondered if anyone could help with a explanation.

I'll bump your post up to see if anyone can answer this.

One thing I would say, stress usually raises blood sugars and the operation and recuperation will be causing stress.

I, like you, are new to taking insulin having controlled my type 2 with medication prior to this. I was started on a slow acting insulin at night, insulatard, and then a fast acting one, Novorapid, three time a day before meals. I started on 4 units before meals and this has risen over the last three months to 24 units before breakfast, 20 units before lunch and 20 units before dinner and my bloods are still high after each meal but quite low before eating at about 4.5. Each person is different and you have to inject enough insulin, as I understand it, to overcome the insulin resistance. My doctor has said that he has one patient that has to take 80 units, three time a day and so it may be that this is what is happening in your case. If I understand it correctly a good balance can only be achieved by trial and error.
