My last test in mid April was 81. I need to get below 54 to have an op on my frozen shoulder. I have been on the lchf diet for 6 weeks but still get the odd high after meals. How long should I leave it before my next test to see a difference as I really need to bring it down.

You would probably want to leave it at least another 2 weeks ideally more to get the benefit of the LCHF I think that the HbA1c usually measures between 2 and 3months depending on how quickly your haemoglobin regenerates. Do you know what you have eaten when you get the spikes?
 
Spikes usually after higher carbs, fruit especially. I am still keeping records before and 2 hours after meals. DN only said check before meals but it wasn't a good reflection as cutting out snacks meant I was having 4 hours plus between checking. As soon as I started checking after meals the bg results went up hence my decision to change from low gi diet to lchf.

Yes you should probably check before meals and then 2 hours afterwards to see how what you ate caused your blood sugar to change (usually rise). If its too much, generally considered to be a rise of 2mmol/l or more, then cut that food out in future. I try and go as low carb as possible, just get my carbs from salad and above ground veg and a bit of yoghurt (also as low carb as possible).
