Paid diabetes questionaire

Anyone else filled this in ? Got email on 19th to say voucher code would be emailed within 48 hours but no sign yet. Have contacted diabetes uk but no response yet

If the email was from Diabetes UK (the charity) them you have to ask them and not Diabetes.co.uk (the message board)
