Please help - crazy BG since yesterday :( LONG POST!

Hello,
I'm really looking for some help as I am running out of ideas. I've been on my pump (Medtronic 640g) since November, and I've had a really really bad few days.
Please bare with me as this is a very long post (and I'm feeling very emotional and fed up about this all so please be kind!). I will try and post everything in the hope that somebody here can help me out. Thank you in advance.

31/12/2015 DAY BEFORE YESTERDAY
9.45am: 9.7 (bolus 5.6) breakfast + correction
12.53pm: 13.7 (bolus 3.4) lunch + correction
1.52pm: 19.1 (bolus 0.5) correction
2.40pm: 21.5 (bolus 2.3) correction
5.10pm: 14.8 (bolus 4.2) dinner
11pm: 12.7 (bolus 8.1) to cover snacks (crisps/nuts etc) that I had until around 1pm (no alcohol)
Bed at 3am

01/01/2016 YESTERDAY
1pm: 20 (bolus 3.1) correction only
2pm: 17.9 (bolus 4.4) correction + small meal
3pm: 18.1 Changed set, but there were no issues/kinks/bends at all in the set (as I tested it once I took it out) (bolus 0.5 - correction)
4pm: 18.5 (bolus 0.9)
4.30: 17.4 (bolus 4.4)
7pm: 14.3 (bolus 12 - meal)
9pm: 5.8
11pm: 14.9 (0.8 correction)
3am: 15.6 (1.9 correction)

Today:
12pm: 15 (bolus 10, correction + meal)
2pm: 17.4
I have now set a temp basal of 125% in the hope of bringing them down. I have also tested my ketones but they're fine.

I'm at the point where I am really hungry but am too scared to eat incase my BG shoot up - I've been diabetic for 20 years and thought I'd have a better grip on everything by now! Looking at everything now, I probably should have set a temp basal on NYE but thought I was just having a dodgy day and things would have settled.
Also, the only difference I've noticed in myself is that I've got a wisdom tooth coming through, and I had a headache from Tuesday until yesterday. I haven't taken anything (other than ibuprofen and paracetamol) so I think that they're just a coincidence.
I'd really really appreciate any help as I am totally fed up and upset.

well this looks like to much carbs to insulin at first glance
may i suggest you test 2 hours after food as this will give give you an idea if the carbs are to much, or not enough insulin i.e. get it down to under 10 on bloods sugars after 2 hours eating food

suggest you up your bolus what are your carbs to in insulin ratio
i am on insight pump have been on pump for 8 years
if i have bad days and i do have stinking cold at present so i up my BOLUS BY 20% and BASEL BETWEEN 125% TO 150% AND on very bad long time high run 200%
BUT THIS IS IMPORTANT . CHECK BLOOD SUGARS BEFORE MEALS 2 HOURS AFTER MEALS BEFORE YOU DRIVE AND BEFORE BED .when it does start to reduce adjust ref pain relief keep to paracetamol/pandol ibuprofen i do not take ref kidney transplant
now being upset and and fed up give your nurse or diabetic clinic a phone talk thing over ,,, diabetes can make you feel
very low now i do not know you but happy new year thank our pumps and just have a walk or talk to friends
let me know how it goes .if not better by sunday down to clinic or phone up the pump rep all will try and help

Thank you @vic hill - my ratios were 1u to 8g so that may be the issue. I'm going to clinic on the 13th so will definitely share my problems with them too. Thank you again.
 
The other part of this is whether your basal rate is correct? Have you basal tested recently?

No, purely because according to the Pumping Insulin book basal tests should be done when BG are between 5.6 - 8.3, and you haven't eaten in 3 hours or bolus-ed in the last 5 - the bolus is the one catching me out as I bolus so often throughout the day to correct my BG. My basal rates are clearly incorrect, it's just trying to find the opportunity to test them is the tricky part!
 
If you are struggling to get to a bg level where you can actually run a basal test, you can run a 24hr TBR at 110% and if that fails to make any impact, go 120%, etc until it all seems to be working again. Then lock those in as your basal numbers and test it properly.

Are you 100% sure you haven't an infection brewing which hasn't made you feel ill yet? Because I find it odd that your basal would change by over 100% at a drop of a hat.
As you are needing more insulin for a basal at the moment then you would also def need to change your carb ratio and also correction I would have thought.

Double check your cannulas on removal to make sure they haven't kinked have you double checked your tubing as well just to make sure you have no air bubbles?

Crisps, nuts etc will also tend to make you need more basal due to the fat and protein content. This wont show until hours after you have eaten them.

Could you have bad vial of insulin? Best way to test this is to inject some from another vial and see if your BG improves as it should
