I am type one diabetic and struggling to get my HBA1c down to 6, I am currently running at 9.6. We are trying to get pregnant but I am worried as I can't seem to get my HBA1c down, my daily sugars are good and I am testing 6 times a day. Anyone who can give some advice please do. Thanks very much everyone :-)

For an HbA1c of 6% you need average blood sugars of around 7.5 mmol/l. Which equates in my experience to fasting blood sugars of around 6 mmol/l and 2 hour post readings of sub 8.5 mmol/l.

If your readings are within this range and you are still getting high HbA1c's then I would suggest that you are running high over night and it may be worth doing some checks at 2am and 4am to see what's going on...

If you aren't running in those ranges then the easiest way to control your blood sugars is to reduce your carbohydrate intake.. and make sure the carbohydrate you do eat is slow releasing.. this will help to stop spikes and will make controlling them easier.

nikki,

A little more information would be handy......for example insulin regime, diet, have you been on a carb counting course etc?

I carb count and have been on a carb coutning course. I wore a sensor for a week to see if I has having hypos in the night and it turned out that I was, so they changed my slow release insuling which improved things but I was still running high in the morning. So I now take my slow release in the morning and a couple of units in the eve before bed. I take 2 unit to 10g carbs. I'm pretty confident that its not teh carb coutning as my sugars are 9 times out of 10 beloe 10, they rise but 2 after meals which is normal for me. I was stable until I started a job where I seem to be stressed alot and have been non stop ill with. The consultant thinks I may react to stress and upset easily and this does reflect in my sugars sometimes. I used to get HBA1c levels of 5.5 but not in the last 3 years :-(

Nicki,

Stress undoubtedly plays havoc with bg and it was the focus of discussion on the forum only a few weeks ago, I know from experience that mine can go from normal to high in a very short space of time in very stressful situations, as you say you are stressed and this is making you ill (which in turn pushes up bg) it's maybe time to address the problem with your current employment to reduce the stress in your life......I know it's easier said than done mind! :(

On a positive note and stress aside..........your Hba1c was previously 5.5 so you obviously know how to control your diabetes, it's maybe time to revisit your notes from your carb-counting course and see if there is anything you can amend to help alleviate your present high bg's, just remember that insulin ratio's do change and it could be that you need to increase your insulin slightly to cover your food.

Hope you get the stress in your life sorted soon and eventually conceive, try working closely with your diabetes care team and don't be afraid to ask for help from them at anytime.

Thanks for the advice guys,

I am in the intensive insulin therapy clinic at the moment so I seem to be having insulin regimes changed every 4 weeks :-(

On the stress note, I have been on courses to help me deal with stress in ways to help prevent high bm's but I just need to try and deal with it in better ways.
