Pregnant: Meal disaster!

I'm currently 28 weeks pregnant, I went to a buffet restaurant, I didn't feel too good before going so didn't eat much. Since then I've been high all day, I have been bolusing all day to bring my sugars down. Last reading was 30 mins ago which 24.1!!:arghh: So I decided to check my pump and realised it wasn't connected properly all this time:banghead:. So although I know the reason for the highs now I'm really worrying that this has damaged the baby as it's been going on all day:(. Anyone have any advice or has had a high this bad? I feel terrible for little one!
 
Don't panic - you've spotted it now. Your baby is fully developed so that high isn't going to affect that. Did you check for ketones?

What's your BS now? Keep hydrated, get your BS back down carefully and stay calm. Be careful you don't overcorrect out of fear.

Can you phone anyone on your team for reassurance?
 
Unfortunately I didn't test for ketones because my Ketone tester has recently stopped working and is asking for some sort of code. I will ask for a new one from the hospital.
Bs is still the same at 24.1 at the moment, I have been sipping water to stay hydrated. I'm not going to bolus anymore, I'm going to drink water and I'm about to go for a walk to see if that helps.

@myarnton1 Im going offline soon. If your BS is stays high, phone triage. The number should be on the front of your maternity notes.

Do not panic! It may be they call you in just for reassurance, or they may want to put you on a sliding scale to get your BS down, or they may check you're hydrated - or they may do nothing at all.

But, thinking about it more, I would phone them and have a word anyway, so you're reassured and so they can check for ketones, etc, if they need to :)
 
Okay thank you! Even though they haven't come down yet I feel a whole lot better, as in my symptoms of being high seem to be going. I will check again in 20 minutes and if they haven't come down again I will phone them straight away!

Good news @myarnton1 :) Keep your fluids up today to make sure you're not dehydrated.

Don't ever be worried about phoning triage if you have concerns. They'd rather you phoned 'unnecessarily' than not call at all.
 
Bs before meal was 8.5 but post meal is now 17.4!!:arghh: I feel like these highs are taking over and I can't seem to do much about them. I think my mother has found a ketone tester so I'm about to go and get that.
I want to phone triage but I haven't got the car today and I don't have anyone to take me to hospital if I need to go:blackeye:

*EDIT* they have come down to 16 now very quickly so I don't think it has anything to do with yesterday and I don't have any ketones. I feel like I am constantly high with few everyday bs in rangeo_O

Did you add a correction on to your bolus and wait to be back in range before eating? Do you pre bolus?

You might as well phone - they can give advice over the phone and if their advice is you need to be seen in hospital, they will help with getting you there.
 
Yes I did a correction dose and I always wait about half hour after bolusing before I eat. But I didn't think of testing again before I eat.
They are coming down now, at 16, I just feel like I'm trying my hardest and nothing is working. I'm constantly getting post meal highs of above 10 even though I wait half hour after bolusing before I eat.

Do you think ratios might need reviewing at this point?

Could you invest in a libre (or would your clinic support CGM) it would help with the sugar surfing - I.e helping to wait until the insulin is working and youre are curving down before eating.

Edit: could you also consider switching to lower GI carbs or fiddling with macros to delay the spike? Obviously being careful to ensure you aren't undermining a nice varied diet in pregnancy
 
Yes you're right, ratios definitely need reviewing. I also have my basal on a 130% increase so it's giving me 30% of my usual basal rate extra.
That's a good idea I will ask if my hospital would fund a CGM.

@myarnton1 Do phone triage if they stay high. If you're not confident adjusting your own basal and ratios, phone your DSN first thing tomorrow.

You're at the stage where insulin resistance increases and it's possible your insulin may need upping.

It would also be worth changing to a new insulin vial just in case that's lost effectiveness.

It's really important to have some way of testing ketones during pregnancy. I had urine sticks as a back up too.

If you're having to correct before a meal and bolus for that meal at the same time, test regularly until you're in range and then eat (always being careful you don't hypo, of course). I didn't eat unless I was below 7. It makes it easier to control your BS and keep in range.

Hi, last bs reading was 7.4 and 6.8 before that.
I have adjusted my basal ratios three times this week already. It's scary how much insulin I am taking for meals etc. I am having to change my pump daily and have asked for a pump that can hold more units because of changing sets so much.
I'm going to ask if I can see my consultant/ DSN more regularly as at the moment I'm only seeing them every three weeks. My bs have been fine in my pregnancy so far and now I think it's really starting to become a lot more difficult and as you said insulin resistance is starting to set in.
I have a Ketone tester now that my mother had at home for me in case of emergencies
 
My basal almost tripled during pregnancy, and my ratios for my meals were greatly increased too. You need what you need. You may also need to bolus more in advance. It really is a big change due to the hormones.
