I was diagnosed with type 2 diabetes six weeks ago. I have been put on maximum dose of gliclazide (4 * 80 mg) and whether I'm put on insulin depends on blood glucose levels over the next three weeks. Unrelated to that I've been seeing an osteopath about a different problem. He has asked me to take something called Reflex Micellar Casein to build up muscle mass (I know now that the muscle wastage and weight loss I have experienced over the last few years was caused by the undiagnosed diabetes). I am concerned that the Reflex Micellar Casein might interfere with the absorption of the gliclazide and so push me onto unnecessary insulin treatment. I can think of two ways it might do this:
i) the osteopath says it forms a ‘slug’ which sits in the elementary canal for a long time; all intentional as one of its effects is to suppress appetite. But possibly the gliclazide might sit in the slug??
ii) if this stuff really is ‘micellar’ the gliclazide could get stuck inside the micelles (tiny molecular agglomerations). Many years ago I learnt about micelles on a pharmacy course and they were being considered as a drug delivery system.
The other factor is that gliclazide already has unpleasant gastrointestinal effects. Having something which parks itself in the gut might make them worse.
