Renewing driving license... help me fill this form!

Hello there. I've been sent some forms for renewing my driver licence. Stuck on a few things. Hopefully someone can answer..

On the back of form D42, it asks if I have ever had or currently suffer from 'Any condition affecting both eyes or the remaining eye if you have one eye (not including colour blindness or short or long sight)'

I'm unsure if I should tick this. I have been getting letters for 6 years (results from eye tests) saying there is early diabetic change/background retinopathy. Does this count as a condition? Should I tick the box??

Also, should I also send my current license to the DVLA? There is a bit under 5b that says 'You should normally enclose your last licence with your application form. However, if your last licence has been lost, stolen, destroyed, defaced or you wish to retain it for the remaining 3 months of its duration, please tick the appropriate box.' but 5a says 'If you have passed a driving test since the issue of your last licence write the new' with a box long enough to fit a date.. so that confused me!

Any help would be awesome

I would think unless you have had actual issues with your eyes then do not tick the box. The standard line It seems is that most people who are diabetic gets the same letter "background retinopathy".

I had the same result with my eyes, but they are perfectly ok and have no issues, for me i would not tell them as I have had nothing affecting them.
 
I agree with BioHazard regarding your eyes.

And of course you wish to retain your licence because it is still valid.

Just be careful you tick the correct boxes as an error can inadvertently give them the bullets to shoot you with.
