Research Questionnaire Help

Hi everyone! I am a graduate student at University College London.

I am currently working on the evaluation and improvement of a website HELP-Diabetes, which is designed for people with type 2 diabetes only, aiming to help them keep a healthy lifestyle. And I need to collect some data through questionnaire.

I would really appreciate if you can do me a favour to complete this questionnaire. This questionnaire would not ask about any personal information, and it is a short one with only 10 questions. I would be very grateful for your help.

So what state secrets of healthy eating are there on it, given that NHS cannot give correct advice!
A non recording "taster" would entice users in. It's health advice, not the masons!!
 
I can wait upto a week fof a non urgent appt with doctor. I am not going to waste his time (or mine) by asking for his permission to log onto a website.
I would strongly suggest that if you want us to comment on this website that either the registration process is devolved away from a busy gp surgery, or that the website is put into "sample" mode to give us a taster of the type of information being offered so we may comment more constructively.

Now I have made the modification on the questionnaire, it would not have a influence if you never used the website! Hope it would make a difference :) I still learning how to design a questionnaire, and it is a good lesson for me! Really sorry for designing a bad questionnaire. What is the website address of HELP-Diabetes? I can't comment on something I know nothing about.

I doubt if I would be allowed to create an account as I don't live in your area.
