Splitting Lantus - is this not supposed to be done anymore??

One of my diabetes nurses told me the other day that her team no longer advised splitting Lantus. (even though I had spoken to another one in January who said nothing...) Is this a known nationwide thing because I've been doing it for many years to get a better balance of insulin i.e. before breakfast and before bed. I have NEVER been told not to do it anymore. I was asking her about a completely different matter so was rather thrown by this. she said it should be 12 hours apart, but why can I not do it by ration of the total i.e. 15/24 and 9/24?? As long as I am covered for 24 hours what is the problem.
 
I split my lantus as advised by my consultant. I don’t do 12 hours apart though as I get up 3/4 days for work at 4.45am and inject then and I’m not prepared to do this on days off. Evening lantus is around the same time 9.30/10.00pm. My consultant knows this and doesn’t have a problem with it. I have not been told any different.

Hi, thanks for your reply. I just think if it works for you then why change it, it is not always practical to do them 12 hours apart but I think as long as you are covered for 24 hours, then what is the problem? Bit annoyed really that the nurse said this when I am doubting if it is helpful or even true!

So, it is alright to take Lantus twice a day, I was never sure to be honest, but it was the only way to try and keep my sugars under control throughout the night.

It has never lasted me the 24 hours it was meant to, usually only 19 to 20 hours then my sugars shoot up during the night.

For the past four or five months I have been taking two doses of Lantus, one at 9:00 o'clock in the morning and one at 9:00 o'clock in the evening. I take a big dose of 30 units at 9:00 AM and then I take a very small dose of 5 units at 9:00 PM and by what you are saying it is OK to have two injections of Lantus in one 24-hour period, is that correct?

Thank you so much for clarifying that. I thought it was OK, but was never sure,

Strangely I brought a pen of Tresiba insulin the week before last, and took my first dose last Saturday, but I did not like it at first, I felt I was having a hypo for 2 hours even though my blood was around 9. Luckily my wife was at home and to sit with me.

I want to try it again next weekend when my wife is home for the day, (She is a teacher), as it is a safe insulin and maybe my body just needs a day or two to get used to it. And then I can inject one less time a day.

I know I should take the same dose as Lantus, which is 30 units in the morning.

Anyway, just realized I am high jacking someone's else's post here, sorry Diabetic Di. I am new to forums.

I will make a new post when I come to taking that insulin again. Sorry guys,

My local team (albeit Australia) still advise that in many cases splitting long acting insulin (Lantus or Optisulin locally) can be a good idea. I’m still on a pretty low dose and so far it lasts the 24hours for me so I just inject once a day before bed.

Generally, I would agree with you that it is always best practice and a very good idea to speak to your healthcare provider when adjusting or changing your insulin regime, but I am personally in a situation where that just does not exist in my part of the world, so I am personally thankful for the pointers this group has given me so far in helping with my diabetes management, especially clarifying to me that it is OK to take two doses of Lantus a day, as I really was not sure until I saw this post and asked a few days back.
