Type 2 Starting low carb

Eating crackers on Low Carb is counterproductive, and crackers. What did you expect to happen? It is not a magic wand, and takes time to work. Every carb you ingest charges up your liver battery so yes, that reaction is exactly what I would expect. Low Carb means Low Carb.
 
PS Gliclazide needs to be taken 20 mins or so before you eat. It does not work after you have eaten. It is not like insulin that can correct for a meal, it gets triggered by the pancreas seeing the glucose levels rise as you start to eat.

How many carbs were in the cracker?

Many of us rise first thing in the morning, even if we don't eat at all. It's the liver dumping glucose in the bloodstream to help get you going for the day.

So you might well be looking at a combined rise from this morning rise plus the carbs in the cracker.

I think we need more info before we can advise, a wholemeal cracker, how many carbs per cracker? Did you have anything with it? When did you take your Gliclizide? When and what did you last eat before the cracker? I was on Gliclizide and I would never expect a rise of that much with just 1 wholemeal cracker - even a carby one of 8g carb, especially the high dose of Gliclizide you are on.
 
It has to be dissolved, absorbed and in place in the Delta Cells of the pancreas at the time when food is first swallowed. This is the timing of the first insulin response. Within 10 minutes of eating it is all over. This is the kick in the pants that triggers insulin release.

Edit to add. Looking at the question of the readings, then errors in measuring could contribute about 2 mmol/l due to meter uncertainty. So a glucose rise of 2 mmol/l from the cracker is plausible. In terms of LowCarb, most cracker run at around 75% carb (eg Ryvita, Ritz) so are not helping an LC and must be portion limited.
