Struggling to find the right Carb/Apidra Ratio

Hi,

I went onto insulin on 15/2 and am struggling to get anything like control.

Initially I was on a 10U Lantus per day and 2U Apidra per 10G carbs.
Liraglutide and Glicazide were stopped.
At weigh in I was 143 kilo, so I'm not a small lad.

That got me an increase in BM over the following week to about 22mmol average. :evil:, and to put it mildly I felt like absolute CR_ _??

Hospital changed my does as follows the following week.
Lantus 15U per day and Apidra at a ratio of 3U per 10G carbs.

This has helped a little. Averages are now
M=15.8
L 20.0
D = 16.3
B= 15.4

Good news is I lost 3kilos that week, and the dietician was over the moon. Average carbs/meal are 50-75 based on her advice.

My question would be is it unusual to be on a ratio of 4U/10g Carbs or 5U/10G carbs?

I know I'm impatient but being so high is making me feel quite yucky. ( tired/ lethargic, blurry vision) Interestingly Urine test was ok. no thirst or drinking excessively.

With type 2 diabetes, you are resistant to insulin, so your insulin ratios might seem high compared with what other people are using, don't worry about this, it will just mean they have less insulin resistance than you.

Have you discussed altering your doses yourself with the nurse? Tell her/him what your levels are, and also discuss correction doses. Sometimes the carb/insulin ratio can vary throughout the day eg you might need more at one meal but less at another.

You need to test waking (fasting), 2 hrs after every meal, before every meal, bedtime.
Are your readings in your post 2hr after meals?

Don't worry about being impatient, your levels are very high, no wonder you feel tired etc.
 
Eureka as a man once said.

More changes to both Lantus Units and Apidra/Carbs ratio and by jolly I now seem to be in the right ballpark.
22U Lantus & 4:1 Apidra ratio.

3 of todays readings are below 8.2 and the ones that were not showed me raising and coming back on target within 2 hrs :D

Hopefully I will soon start to feel bit better, and some of the funny nerve sensations will subside.

Now the cometh the hard part:

i) Staying there
ii) Avoiding Hypos
iii) Not being so irritable

Diabetes - Not my chosen way of life, but you have to play the hand you are dealt!
