Thinking of splitting Lantus

I've been doing some basal testing recently using my libre. For background I have an HbA1c of 6.2, which I'm very happy with, but as my husband and I are trying for a baby my fasting levels ideally need to be between. 4 and 5.3 - and I currently wake up between 7 and 8.

My main problem is the evening and overnight - at the moment, taking 10 units Lantus at 7am I am fairly static all day but will then start dropping from about 5pm - if I don't eat an evening meal I will go hypo. I then get a rise around 11pm to 2am, and then a slow drop overnight but not very much. This info is based on several tests, so not a one off.

If I increase my Lantus then I'll have more hypos in the evening. So I'm thinking it might be better to split it and smooth it out? As I'm only on 10 units is 2 injections of 5 feasible? Do they need to be exactly 12 hours apart?

Any advice or experience appreciated.
 
another thought might be to move the lantus dose to evening -- as you state a 5pm drop this is 10 hours post lantus dose
if you take the lantus at circa 9pm this will coincide with a 7am wake up.

if you do decide to split make sure to take approx 1/2 dose in the morning you start -- don't start the split in the evening after a full morning dose :)

the 12 hours is not critical but is worth noting as is keeping to much the same times once you do split

your libre will guide you as well !!
 
When I was on split lantus I found my control was far more accurate though I applaud your current results. Many a baby have been carried and born healthy on those numbers! As said start the split in the morning. Some people will find a 50/50 works but others will not. I would try to start on 50/50 and if you find highs and lows appearing then adjust it accordingly. I always found my lantus at night more weighted than my lantus at lunch time for example. 12 hours works for some but a lot of lantus users take their insulin before bed and at lunch time. There will be a period when the two injections overlap. For some, adjusting the doses covers that and settles it up. This negates the need to get up at 8am every day to take insulin for example, if your first dose is with your lunch at half 12. My lantus was always between 12-1 and 9-10 at night. It is a very individual thing. Good luck to you :)
