Type 1 Thursday crash!

Dear All,

I am a relatively new Type 1 diabetic and would be grateful to hear your experiences.

I have been very lucky to have a really good diabetes team locally who have helped enormously getting my health back on track and in general my BG now seems to be under good control.

I have a persistent problem however; my blood sugars often dip to hypo at some point on a Wednesday and then go high on Thursday and wipe me out for 24 hours.

I have a high stress job and the above happens when I'm working only. At Christmas I was fine. I am getting better at "anticipating" the hypo, but that actually worsens my BG on Thursday. I eat well and (have always) cycle.

Other than changing jobs, does anyone please have any suggestions?

Do you Do anything different on a Tuesday e.g. longer bike ride? That could hang over into Wed.
Otherwise you could try a few more carbs on Wed to avoid hypo territory. ARE you over treating the hypo and taking you too high?
Then on Thursday try to keep to meals that you know work.

It's pretty hard based on the info you've given to analyse why you are crashing on a Wednesday in particular - are Wednesdays particularly stressful for you ? Working in a stressful job will involve additional hormones - cortisol and adrenaline, both can adversely affect your BG control, apart from extra careful monitoring using a CGM device such as a Libre sensor will help as you can see which direction your BG levels are heading.

How do your BG levels look in the lead up to the crash and can you test more often to check ?

I'm not aware that I do anything different on Tuesdays. Tuesday is the one day in the week when I never cycle. Wednesday is typically no different in relation to work type or stress.

On Wednesday early to mid afternoon I typically dip to between 2.6 to 4.0 when any other day I am probably around 7.0. I correct with a small carton of orange juice and around 10g of carbs. My bloods are in normal range for me in the morning (see below). I recognise the symptoms of the hypo starting and can normally catch it at the high 3s. I feel terrible afterwards however, probably for a good two to three hours; light headed, can't think straight and absolutely no balance!

On Thursday morning my bloods are somewhere between 14.0 to 16.0. Ordinarily they are between 9.0 & 10.5. They were 15.0 this morning. Bloods were 8.8 last night and I had a normal meal that works for me and cycled for 45 minutes.

I am on 32 units of background which I take in the morning and then 6-6-6 on quick acting.
 
Have you spoken to your Dsn? Your basal seems high compared to your bolus. And are you matching carbs to those fixed doses? If you are going to bed at 8 and getting up at 10 then you may be experiencing dawn phenomenon or you might e hypo during the night and liver dumping. Without overnight testing you won't know.
