Hi all, new to the forum (but not tto diabetes, type 1 for 33 years diagnosed at 10 months old).

I am wanting to get tighter control of my sugars, my last hb1c was 7.5% and wanting an improvement in this. I usually have a salad and an apple for lunch and have 3 units of novorapid, my post 2 hour glucose is about 170, i have tried bolusing 30 mins beforehand and this doesnt seem to have improved things. I am just wondering if to cut out the apple completely as i feel that this must be causing the spike as surely salad cant! Do any of you have any recommendations in order to stop this spike (i have tried 4 units but then i go low and 4 units for an apple seems a lot to me)

Sorry for what may seem like a simple/silly question for someone who has been diabetic for over 30 years. I used to live in the UK but have recently moved to Italy for my husabnd's work and i am feeling a little lost regarding help for my diabetes, i do have a diabetic dr here but she doesnt speak that good English

if it is, then are you going back down to target levels by evening meal time?

Hi, thanks for the welcome , yes i seem to (mostly) go back down to normal before my evening meal so i am assuming this is all ok, would just like to stop the spike and i can only assume its the fruit that's doing it. Sometimes just wonder if it would be easier to eat no carbs at all (sorry having a bad day today with it all)
 
your right in your thinking about getting the insulin in earlier to prevent a spike........don't be afraid to experiment further with the timing.............increase it, although you wouldn't expect the need for more than 30 minutes with a salad and apple....

what is your BG reading before lunch?

I use mmol/l as opposed to mg/DL but your 170 works out at 9.4mmol/l, which isn't that bad at all, single figures from a pre meal reading of say 5/6mmol is quite good actually......

its probably worth doing a basal test from lunch till dinner............
 
How would i do a basal test?

I used to work in mmol but had to change when i moved to Italy as the dr didnt understand mg, something else i had to get used to!

My reading before lunch is usually around 100-120
 
Hi all, tried to cut out the apple today and had chicken and a packet of baked crisps, pre sugar was 6.4, had 3 units for my crisps, i hour later 6.7, then 2 hours later 11.7, had my insulin half an hour before my meal, i just dont know what to do in order to try and correct this, also gave myself an extra 1 unit of lantus this morning but that hasnt seemed to help either, im just loosing the will here and ready to give in!
 
If your continuously high by dinner that suggests your ratio need adjusted........

but it also could be some kind of delayed digestion going on......gastroparesis, this seems ot be quite common among diabetics......

but I would try evaluating your ratio at lunch before going down that road.....

you previously asked about a basal test.......to do this you want to have a carb free meal or even miss the meal, in this case lunch, you would then test at lunch as normal, then again mid meal, then again at dinner.......

if your basal is right, you wont move by any more than 2mmol up or down............if it does go more, you need to adjust......which is very difficult on a single basal dose.....
 
My sugar is perfect all morning and then comes lunch (but i have my basal at 11am. i dont have many carbs as it is for lunch (just an apple or a packet of baked crisps) but will not have those tomorrow and just have salad or chicken to see. I am on a double basal (in a morning and at night).

Thanks for the advice, will see how i get on tomorrow :)

Hi

So i have tried the no carbs at lunch today and found my sugar has slowly been rising during the afternoon (nothing major as such) from 6.4 to 8.9 and then started going back down again to 5.4 so thinking its nothing to do with my basal rate, time to increase the fast acting - all for an apple or a packet of crisps, tempted just to stay eating carb free for lunch as it seems easier!
