Tresiba Dosing

I’ve recently changed from Toujeo to Tresiba insulin. Toujeo never seemed to work for me, my blood sugars were up and down and I would always be taking correction doses to get them down. I was on between 38-40 units of Toujeo taken in the morning.
I started Tresiba about 2 weeks ago, taken in the morning and I’ve never had so many hypos since being diagnosed. Initially started on 38 units, but noticed quickly it was too high. Over the course of the two weeks I have lowered it to 32 units but it still feels too high.
I always seem to have a hypo early morning, as low as finger prick 3.3. But then it’s level through the day, then it seems to peak around 6pm where I drop back to low 4s.
Should I gradually keep lowering my dose every 3 days to see if it stops the hypos?

for tresiba, changing the dose once every three days is too often. This insulin is looping and the full effect of the dose change can be observed only after 5 days. When I changed my basal insulin to tresiba, I was told to reduce its dose by 20%, and then adjust it until bg becomes normal

I notice you're not an insulin user. It would be impossible for the NHS to cope if everyone with type 1 diabetes consulted their healthcare provider before making insulin adjustments. Apart from when you're just starting on insulin therapy, we have to adjust it all the time depending on the 42 things that can affect bg levels.

Depending on how tight control your care team want you may need to talk to them about adjustments

My care team knew me and my knowledge level so said go and adjust as I see fit

If you fall in that latter category then yes adjust as you se fit.. every 4 days or so lower the dose I was always told go 2 units at a time so generally stuck to that until I wanted finer control

Have never consulted anyone about adjusting my doses and have been at this 10 years now. If I want to adjust I will.
As regards Tresiba it is definitely an adjust and wait 3-5 days before adjusting again. If you can get to the sweet spot it’s a great insulin.

As regards Tresiba it is definitely an adjust and wait 3-5 days before adjusting again. If you can get to the sweet spot it’s a great insulin.

It’s as much a way of saying:

We here are not qualified medical professionals and therefore can not give medical advice

However ‘what I did was…’. And therefore what you wish to do is upto you
