Type 1 Tresiba Switch

Over time my body has called for increasing amounts Basal insulin - I currently take 32units Levimir A.m + 32units P.m!

Having read a lot of posts on this forum my diabetic nurse has agreed to change my Basal insulin to change to Treshiba.

Considering to my lifestyle, average weight and general well being she was quite sure I was not experiencing Insulin resistance.
(I am 44 years old, been Type 1 Diabetic for 30 years and have very good control - last result 45Hba1c).

I appreciate we are all different and that only with experimenting, perseverance & testing we find the answers however I would like people with a similar experience to advise on the amount of Treshiba they started with and also whether you administer Treshiba A.m or P.m?
 
I switched from Lantus to Tresiba nearly three years ago. Initially I took 20% less and then made adjustments from there until I found the correct dose. The idea of taking less when you are changing basals is to avoid night hypos. I just carried on taking it as the same time (10pm) I don't really think it matters what time you take it as there is no peak that you would notice once you are up and running.
 
I switched to Tresiba 6 weeks ago as I kept having higher results than I wanted in the morning, despite low carb (average 8-11). I was on 18u of lantus AM and PM - the split dose worked better. DSN offered tresiba, with 20% less as @robert72 also mentions. I'm now taking 30u of tresiba in the morning and it has made a massive difference. I started on an evening dose and kept having hypos each morning, so after a bit of trial and error, I switched to an evening dose and now regularly get morning levels between 4-7. I've had type 1 for 25 years and latest HbA1c of 44. I know that this will drop at my next blood test early April thanks to the tresiba. I don't know why I was getting the morning hypos as it shouldn't make any difference what time of the day I take it, but for some reason for me taking the tresiba in the morning makes a difference.

Just got back from seeing my diabetic nurse who is now trying to authorise Tresiba for me.
I was on 45 Lantus for many years and switched recently to 30 Lantus AM and 30 Lantus PM, but it's just not working for me. Despite low carb diet my numbers just rise and fall all day.
I'm hoping for good things with Tresiba.
 
I love Tresiba now.

Really flat up to 4am. Then it cannot cope with my DP and symogyi effect. Have to have a bolus at 4am of just 1 unit and another 3.5 an hour before getting up.

I dont like having to do this but have no choice and at least my levels dont vary up til that point.

I have mine with my 2nd bolus before getting up..

I have had to up my bolus given for meals though..

Overall though I am really pleased not to worry about hypo's pre 4am. Thats a first in 30+ years!!
 
Well I picked up my first box of Tresiba this afternoon. As I was on about 60 Lantus per day, split morning and evening, my nurse suggested I start on 50 Tresiba and adjust every 3 days. To be honest this amount does scare me a bit. Does seem a lot? Other people in the forum seem to start with less than they were on before... I guess it wouldn't hurt to start smaller and build up if need be?
 
Since it's so long lasting I know it shouldn't make a difference when you take it but like peppergirl I also find I get a much flatter profile if I take tresiba in the morning. Don't know why. I also find it's pretty flexible to exact timings. By that I mean it doesn't make a difference to my daily profile if I take it at 6:30 am during the week and 9:00 on the weekend (so thankfully no need to set the alarm on Saturday ). Good luck with it!

Thanks for that. Lantus just didn't seem to work for me, it's like I was just relying on my Novorapid. Fingers crossed for my Tresiba
