Hi all,

Im after some advice.. I was switched to Tresiba (24u in the PM) around 2 years ago after being on Lantus for 10+ years, and at the time my hba1c was 7. Since then, my hba1c went up to 13, now back down to 9.8 but around 3 months ago I was moved to Levemir twice daily (18u in the am and 15u at night), as it was agreed I needed a split as my sugars do creep up in the morning due to the dawn phenomenon. It was working really well till I got back into the gym after 6 months out, I had night hypos for 2 weeks, and when I stopped taking my night time background they still went low! So once daily Levemir, 16u in the am would send sugars high without exercise, night time hypos if I did exercise. So it was agreed to switch back to Tresiba, with no real guide as to how many units - I went for 15. I have attached an image of what my Libre shows - my humalog has the tinyest effect which is where the little dips are but nothing is working at the moment - other than gym. They plummet post exercise then shoot back up again soon after. It seems Tresiba is having no effect - but maybe I need to up my dose? How long does it take to kick in as I had previously been told it can be 7 days before you see effect? Can I do both Levemir and Tresiba? HELP!!! Im hoping to move to a pump soon but I have diabetic retinopathy in my right eye which I have had numerous laser treatments and also had the Eylea injection privately - it is much better but im so worried with the highs! I know the best place to ask is here :)

I have found Tresiba to be really good (much better than Lantus). If I alter my dosage I usually wait 3 days to get an accurate reading
 
Hi @Heatherlou18 I agree Tresiba was the best basal I tried before I got a pump, it took me a few weeks to stabilise so try to be more patient and as Porl69 has said wait 3-4 days before making any changes as it lasts 36 hours so you need to see it settle down first before diving in with adjustments. I was started on a dose about 20% less than levemir and gradually tweaked my tresiba dose up until it came good.
 
Forgot to say. I was on 22 units of Lantus when I started on Tresiba. Am now on 11 units of Tresiba after 2 1/2 years.
