Type 1. 3 months since diagnosis.

Recently I was diagnosed. In the last few days my glucose has been high waking up (9.7-10.4). Taking 6 units of lantus in the morning and the. 2 units per meal. My nurse still thinks my pancreas is spurting bits out here and there. I have no idea on what to do ? Do I need to adjust my lantus?

I take it that you haven't been told the basics of how to adjusts your doses, or that you are not 100% sure.

It is hard to know how to advise on whether you need to adjust unless a little bit more info is given.

Ie. What are your levels during a day at the moment? As adjusting lantus could impact on these levels.

What time do you inject the lantus?

What time is the last time that you eat pre bed?

Do you test at 3am at all?

It might be worth taking your Lantus at bedtime, in case it's tailing off a bit towards the end of it's 24 hours.

I take my lantus at 7am sharp every morning. My glucose through the day varies. Mostly between 5.5-10.4
The last time I eat before bed is about half 10.
I have been told how to adjust. It's 1 unit for every 10g of carbs I eat but I don't feel confident enough to adjust incase I make myself worse. Also I wanted to ask, is sweating constantly a bad sign? X

P.s I don't test at 3Am xx
 
Hi there.

I take my lantus in the evening and use my waking test as an indication of how well it's doing. I was told by the hospital care team and confirmed by my diabetes nurse that adjustment of your long acting should be done one or two units at a time and then left for two or three days to see the effect. Adjusting up a unit should be ok & then see how it looks over the next couple of days.

My insulin doses have fallen steadily since I was diagnosed four months ago, generally in line with my weight loss. I'm also suspected to be still producing but in a steadyish state, as opposed to fits and starts...

Show your bg readings to your DSN and they will advise on adjusting your insulin doses, but do keep in mind that the first few months can be hit & miss bg wise so don't get too disheartened, once you've been diagnosed for 6 months you will be eligible to go on a carb counting course such as DAFNE.

The sweating I'm not too sure about but it is a symptom of low bg, however given the warm temperatures over the last few weeks I think it's safe to say we are all sweating in the heat so your not alone, if it all concerned then mention it to your gp or your DSN when you speak with them.
 
It may well be better to take your lantus at night.

Lantus does not stay in the body working for a full 24 hours. So uf you are taking it in the morning, by the time you wake up it has probably worn off.. That is why I suggest checking your blood at 2 or 3am to see what your levels are then.

You need to talk to your dsn about this, as some people get dawn phenomen which can also cause morning levels to rise.

It maybe that your dsn would suggest splitting the dose so that you take some in morning and some at night. However your dsn would need to see what is happening at 3am to establish what to change.

If you took your lantus at 10pm each night, it would probably be working less by 5pm (however everybody is different on this) and it maybe that your evening meal bolus ratio may have to give you more insulin to cover those 5hours ish when your basal insulin is not working or less effective.

I would suggest a few days at checking 2am or 3am kevels and speak to your dsn.

Lantus starts to act within 1–2 h of injection its peek is about 6 hr after injection effects end about 18–26 h after injection and between 5–10 h after injection is when you are more likely to experience being low (according to my endro). Some users do not get a full 24 hours of action from Lantus and may also notice more peaking in activity and a tendency to have lower readings about 6 hours after the injection. A larger peak in activity at around 6 hours is usually associated with a shorter action time, and vice versa. Splitting the dosage and setting it 12 hours apart isn't uncommon to take full effect of the drugs purpose. But like all things talk with your doc about what you are doing with it before doing it on your own.
 
I checked myself at 4:30am and I was high at 9.7
So I'm going to take my lantus at bedtime and see how it works out. My brother is type 1 too so he's helped me today work out the issue. :-). Thank you everyone ❤️❤️ xxx
