weight and high blood levels

My wife is touching 80 and had been diabetic for 20 years.
She has been injecting Novamix 30 for about 5 years.
Over time her weight has increased and her feet are beginning to swell. Our new medics don't seem to have much idea about alternative medication.
In the past Metformin and Gliclazide have proved counter productive.
Any suggestions will be helpful.
 
How good is her blood sugar control e.g. what was her last HBa1C? If she doesn't mind one or more extra injections, then the Basal/Bolus insulin regime will give much better sugar control and allow her more flexibility in meals thru carb counting. Also Basal/Bolus will allow her to reduce carb intake which will reduce weight gain. If you're not sure what this means let me know. Discuss the possible change with your nurse.
