Sugar level off the scale, 2 hours after breakfast.

Evening sugars levels, between 7/9. Pre breakfast, 8/10. Two hours later, blood sugar level is 22+....

Insulin taken at breakfast, 12 units insulin glarine, from Lantus Insulin Solostar and 8 units insulin aspart from Novorapid Flexpen.

I have 45 grams of carbs for breakfast. When I increased my doseage, within 2 hours im hypoing, if I then decrease my doseage, well you know what happens......

Any advice, would be much appreciated.

How long before breakfast do you inject your bolus and as a matter of interest what did you eat?
 
Are you eating cereal? Most have a high GI and spike your blood sugars, then drop quickly after. Your Novorapid works for four hours so may well be active after your carbs have 'run out'. I used to have the same issue. Ran very high for 2 hours then hypo an hour later. Stopped eating cereal and bread and it cured it. I suggest testing your BG each hour after eating and see the effect.
 
Dawn phenomenon is an increase in blood sugars first thing in the morning. In a non-diabetic it isn't a problem because their insulin sorts it out. In us diabetics the effects vary person to oerson. Some don't have it (lucky *******). Others it starts around 3am. Others as they get up. I correct for mine, I'm also in the pump to deal with it.
But I wouldn't eat 45g of carbs in a day never mind for breakie.

As CarbsRok says work on your injection timing, here's a great article from Gary Scheiner (Think like a Pancreas fame) called Strike the Spike which discusses bolus timing:

My morning breakfast is 50g of carbs and I bolus 20 mins before which keep my bg levels below my bg target range, you may find you need to inject further or less ahead of your breakfast depending on your meal.
 
Hi John how long have you been diagnosed? Looking at your blood sugar readings I think you need to give your diabetes nurse a ring as it looks as if your background insulin needs adjusting. 10 mins is possibly not enough time for your insulin to start working so it peaks when the carbs hit your blood sugars.

I inject 10 minutes before eating and i eat 2 Shredded Wheats and a one egg omlette.

I have to inject much earlier for breakfast than for any other meal. When I was on Humalog, I had to inject up to 45 mins before.

Try keeping your insulin the same for now but try injecting Your fast acting meal insulin earlier. Move your injection very gradually and always proceed cautiously.

Edited to add that I have a large bowl of granola, bran cereal and full fat milk which adds up to a similar amount of carbs as you have.

Hi CarbsRok, 4 months. My diabetes nurse told me it was fine to eat within 5 to 10 minutes. Maybe its just me..... I'll give the long acting insulin a look at. Many thanks....

Hi John, you really do need to look at your bolus timing when not feeling so overwhelmed by it all read other posters experiences about the bolus timing.
There's a couple of books that might also help you out, think like a pancreas and also using insulin both can be found on amazon.

If you always get up in the morning with a bg of 8mmol or above, then ideally you should be giving a correction bolus to get back to a good bg target of about 6mmol. Look my msgs on the 100 rule etc. Also, regardless of whether your basal is at the correct level or not, eating cereal containing a lot of carb, usually makes bg go up by a fair amount which is why people bolus in advance to try to minimise the rise. What you could do tomorrow morning is inject the same amount of insulin with a correction and then eat 1 shredded wheat with milk instead of the 2. If eating the smaller carb works ok with the insulin and bg levels, then you could use that dose to eat whatever amount of carb there is in 1 shreddie with the milk.
 
Hi @JohnEverton , another thing to consider is if you're injecting lantus in the morning , it may have ran out before the full 24 hrs and then may take a bit to get going after injecting, which might make the dawn phenomenon worse. I'm on lantus and novorapid and I inject the lantus before bed. As others have said I tend to inject earlier for breakfast than I do for other meals, normally 30 - 45 mins before. ( 2 slices of toast )
