Pump: Delayed rise

Hi there! Wanted to get your take on this. The last two days I've noticed my BS start to spike at around 3pm. Today for lunch I had a wrap with tuna mayo mix, salad and a bit of cheese. My BS stayed in the 4s after eating this until 3 hours later when they started to creep up. I've corrected since then with 2.5 units of insulin and its stubbornly sticking in the 8s. I've noticed this for the last 2 days - exact same lunch both days. I changed infusion set and insulin last night (I'm on a pump) and basal tested this morning where I stayed in the 4s all morning so it's not the infusion set or cannula as I've checked this all today. My question is could the wrap at lunch be slow to digest with the tuna/Mayo mix? It is prob partly basal too but I've never known it to be so stubborn when basal needs increased so wondered if that played a part too but not sure so hoping someone might be able to help...thanks in advance!
 
Fat can delay a BG spike. I'm unaware of how much fat would be in what you've eaten, but there certainly is fat in the tuna, mayonnaise and cheese.

It's driving me mad. I guess the only way to know for sure is to have something else for lunch tomorrow that I'm familiar with and would know the digestion. It's only the last 2 days so I'd say it's a case of basal needing increased and maybe something to do with the fat content of the lunch too. Never a dull moment with diabetes! Thanks for the quick reply :)
 
Was it more protein than usual? Protein will spike me if I eat more than a few ounces. I do find fat to lower and slower a spike. Was the wrap lettuce or grains?
 
Hi @Kristin251 I have to admit I'm not great with the lchf side of things which is why I'm wondering if this has had any impact. I know the basics, that fat and protein will delay the rise and can take up to 8 hours to digest if high fat etc but that's as far as my knowledge really goes on that front im afraid. I generally eat about 150g carbs a day maybe more so it was actually a bread wrap and about 30g carbs (I bought it from a local deli so that's my estimate on the carbs). I would imagine a big part of it is probably basal adjustment but I've just never seen a basal change needing as much correction as this except while pregnant (which I'm definitely not lol!)
 
I would try cautiously increasing your basal if the problem persists. I find wraps can be challenging but I need to watch for highs within the normal two hour range. To deal with this, I usually add on a little extra to my bolus.

A delayed rise sounds like basal possibly. I'd try a different lunch to be sure, but then look at a very small increase and monitor closely. A while ago, I started going higher than usual late afternoon and early evening and I had to increase my basal. I quite often need to tweak my basal so perhaps it's a combination of the wrap and needing more basal?
 
Hi @azure yes I'm thinking the same too, a combination of both. I'm glad someone else finds their basal needs tweaked a lot too, I was starting to panic as I've been tweaking it across different parts of the day and as one is sorted, another starts playing up. I also have a bad habit of comparing my control during pregnancy to now and in also recognising that id all the time in the world to pour over all the various reasons and tweaks before baby where now I don't quite have as much time lol! I've upped the basal for tomorrow afternoon so will test and sit tight and see what happens tomorrow. It's non stop fun this diabetes game, even after all these years!! :rolleyes: So glad I joined the forum though, it's nice to not feel so alone with it when things are playing up!

Better still why not have a low-fat breakfast then skip lunch and see how your bg levels behave in the absence of food at lunch, if your bg levels go up then you know it's a basal rate issue.
 
Hi @noblehead I re-read that earlier and thought it was daft, don't really know what I was thinking! I basal tested this morning and all was well so will swap and have breakfast and skip lunch for an afternoon basal tomorrow. I must just need a complete overhaul with my basal this week :rolleyes:

Yes that's what I would do, if your bg levels remain steady in the afternoon then you know it could be down to delayed rise from your lunch-time meal.

I quite enjoy a wrap but usually have a chicken filling with mayo & salad, must admit the fat content isn't enough to effect my bg levels later on but we are all different as we know.

Thank you! I'm thinking it is more basal than anything as normally this kind of lunch wouldn't have an effect on BGs but I'll know one way or another tomorrow!
 
Simple solution is to skip lunch and do a basal test.
