Just about to switch from Lantus to Levemir

Been diabetic for 36 years, taking Humalog as short acting and Lantus as my basal. Have been on Lantus for around 10 years.

My results could be a lot better, typical bg seems to be around 8/9. I generally find my mid morning BG to be at its highest then, and evening BG are usually not too bad. On going to bed most nights as I take my lantus my BG is around 6. Next morning typically it will ruise to around 8

At the moment I take 32 units of Lantus usually around 10pm. DSN suggested i split the dose 18 in morning and 14 at night and to monitor progress

I do pretty much stick to a routine in that Mon - Fri I go to bed around 10pm and take my jab then. I get up around 7 am and take my Humalog then.

When would be the best time to take the levemir. DSN advised as long as there was over 7 hours between them there should be no issues

Do I take Levemir at 7am then 10pm, or would I be better taking at 7am then around 6pm when I have my evening meal and Humalog

I would appreciate any advice from anyone taking Levemir

I take my Levemir only once a day despite it not lasting 24 hours. I just adjust my Bolus ratio to suit and it works out fine and saves an injection. I take it at bedtime leaving it on the pillow so I don't forget or confuse it with the Bolus

I changed to Levemir a few months ago. I take mine at about 8 in the morning & 8 in the evening
I've found it to be more consistent than Lantus, glad I made the change.
Hope it goes well.

Many take a 12 hour even split, some require more during the day, but equally some require more at night. It really depends on you and your lifestyle.

Some (like @wannabemammy) do a sort of staggered approach where the evening dose is taken just before bed and the morning dose upon waking. This can help slightly with the dreaded effects of DP.

It's a fairly flexible insulin so your timings can be adjusted with ease. But ultimately it's what gives you the best BG profile:)

Been on Levemir for about 2 weeks now 16 in morning and 16 before bed and its still not going that great to be honest.
I inject Levemir around 7 am then again about 10pm. Where I am finding the biggest issue is my late morning just before lunch levels. I am taking around 2 units of Humalog to 10 grammes carbs.

As an example my 7am was 6.5, good so far, so i take about 40 g of carbs in form of porridge and semi skimmed milk. Around 10.30 |I have acouple of digestives about 20 g of carb and yet by 12.30 my levels are at 11.5. That is despite taking 16 units of Humalog.
Lunch is about 50 g of carbs with, 14 units of Humalog and by 6pm I am still around 11.5.
Dinner is 50 g of carbs around 6pm with 16/18 units of humalog and a biscuit around 7.30. However my levels are dropping to 4 around 10.

Totally confused, where am I going wrong

Best thing would be to do some basal testing - you can't really sort out bolus ratios until your background is level.

@smc4761 I was on Levemir 24iu x2 and was finding that at that dosage level ie .2 iu/kg it was quite peaky at 6-8 hrs and then dropped off. Have a google for the Levemir action-profiles to see the graphs.

If you do basal tests you may be able to evidence this to your Dr. Solution for me has been to switch to Tresiba which seems to be working a lot more consistently for me.

Ps just noted you are only two weeks in so would also suggest stick with Levemir a while longer until you can evidence if it is a problem.

I have levemir, I started off at 12 units at bed time and on waking, but my sugars were still high, I'm now on 16 units and my BG is pretty much always in target (4.5-7.5) but I'm only a month old diabetic, so could still be in the "hunny moon period" as they say :S but I have no comparison, sorry

Also, my diabetic nutritionist said not to worry about doing the levemir at set times, (as i was told by my diabetic nurse on diagnosis) but just to do it as soon as you wake up and just before you go to bed...
