Hi, I've recently been changed to tourneo? from lantus as my bloods were low in the morning, now after 3 weeks my bloods are generally high teen to 23/24, I'm using a lot of apidra at each meal (6_9) whereas with lantus ( twice daily, 6+6) my bloods were generally below 10 and only used 2, max4 units of apidra (rarily though) is it sage to stop tourneo and switch to lantus tomorrow? My consultant told me I didn't need to wait for instruction from the diabetic nurse to change insulin dose as I have a good handle on it, so question does anyone know if its sage to change straight away because as it is I'm open to complications alsoy eyesight and balance isn't good as its so high, thank you for any help you can offer, Gary

Hi @Gazz77,
Toujeo is a product of a US drug company called Sanofi.
Toujeo is glargine, the same insulin as Lantus, but from different manufacturer.
Under their product info products.sanofi.us, it mentions that the concentration of Toujeo is 300 IU per ml ( compared to Lantus at 100IU /ml). I imagine your nurse explained this to you.
BUT in the blurb comparing Toujeo to Lantus it mentions that the the dose of Toujeo needs to be higher than the equivalent dose of Lantus (and quotes some study about this). It does not name the dosage difference.
Certainly if you are taking less than the equivalent of Lantus dose with this new insulin is it any wonder you are needing extra insulin such as your Apidra ?
Perhaps you could point this out to your specialist and also note that this is something discovered despite your eyesight probably not being as good as his/hers !!

Are you wanting to know if you can go back to the Lantus or if you can adjust the new Toujeo....?

I have no personal experience with toujeo but in the absence of that info I would start on the same doses as before and then adjust as needed with basal testing..

Hi, I'll start on same dose of lantus and change it as I need but the Dr told me to take 6 tuojeo, I've increased to 10 with not much change but just read on other site to use 20% more toujeo compared to lantus so I'll try it tomorrow, thanks for all replies, take care

Hi @Gaz77,
apologies for not replying sooner. In Oz it is 6 am!!
So it sounds like your doctor was being careful and conservative in his advice about the changeover dose.
I cannot give you advice but just state what I do: I used to increase my long-acting insulin by 20% at a time and usually allow 2 days before increasing again to allow time for the first dosage of long-acting insulin to 'settle out'.
But please do your adjustments in step with your doctor's and nurse's advice. The 20% plus of Tuojeo over Lantus dosage you mentioned as the suggestion from another site might suit one person but not another.
Good luck !!.

Actually yes, I should've realised from my past experience what's suits one may not suit another, I will adjust my toujeo to 12 for 2/3 days to see how it goes, the Dr initially said 6 but over the past 3 weeks ive increased it to 10, today first day at 10, so hopefully the next few days I'll see an improvement, thanks for your reply, have a great day
