Hi guys, as I mentioned many times I switched to lantus a few days ago, I took 15 unit at 10pm.. noticed that after at 4 pm (18hours after injection) if I eat a small thing my blood goes to 200 something.. does that mean lantus only stay 18 hours in my body?

if that true, if I didn't eat anything from the time that lantus leave my body until next injection (6 hours from 4pm till 10pm) will my my blood glucose will be going up or just stable since I didn't eat anything?

am considering changing my dose to 6am so when the lanuts is over after 18 hours i will be sleeping not eating anything during that time.. but first please guide me about the above point is my blood glucose will be rising if the lantus is over even if I didn't eat anything?

Hi @azizdhl, Lantus is a long-acting insulin used to provide a basal supply of insulin to the body.
It is not intended to cope with food. That is the purview of short-acting or bolus insulin which is usually prescribed for use before each meal and sometimes before a snack.

hi @kitedoc

you didn't get my point, I know that lantus doesn't have anything with food covering.. but I mean that I am suspecting that lantus doesn't last more than 18 hours in my body, so if I eat a tiny thing like 10g of carb is that possible my BG would go from 120 to 250?

so I was asking if it was confirmed that lantus doesn't last for more than 18h in my body, if I didn't eat anything for 6 hours my BG would be stable or the lack of background insulin would lead to increase my BG?

Its a tricky one to answer.

To the first one about eating 10grams of carbs, the example you give seems a little on the high side to me, I guess it depends on what type of carb it is, but in the long run yes I guess it could make you rise from 120 to 250.

In theory, if you don't eat for 6 hrs your BS could stay level, but the body does things, sometimes, beyond our control, so if you meditate for 6 hrs then maybe they may stay level but watch an exciting movie and your BS may change.

I've been on Lantus for a long time and have in the past had night time hypos, but now I split mine. Currently I take 19 units in total (this may change as I'm experimenting with doses) so go bed in goes 11 units, when I wake up, some 8-10 hours later in goes another 8.

Lantus is, is as you know, is a background and works in combo with other insulin so to control things. But as with all things, no one rule fits everyone and for a while I have realised that it does last as long as people say in me.

That is just me, I'm not medically trained and the info above is based on my own observations.

when i was on lantus i found that it only lasted around 18 hrs i used to inject about 10pm and it would wane off around 5pm the next day i am now on tresiba which has worked out much better :)

It does not last 24 hours and this is a very known issue with Lantus among users. Changing your Lantus shot to 6 AM will only make things worse because you will have no insulin to deal with dawn phenomenon. Insulin must be available in your body 24/7 otherwise your sugar will rise regardless of eating fasting sleeping or even exercising.

I heard a lot about this dawn phenomenon but in 18 years I have never been high late night.. does it necessary happen to all type 1 diabetics?

so if I sleep at 80 and this dawn phenomenon happen to what's the range that I may wake up with?

have you tried any solutions before moving to tresiba? like change the time of injection to 6am or split doses every 12 hours?
