The thought of changing my lifestyle feels like climbing a mountain and I keep procrastinating. Any tips from people who have been successful implementing small changes gradually and been able to stay motivated and consistent?

Considering you don't have diabetes but are worried about developing it, unlike most of our members who do have (pre)diabetes, why not cut back on the sweets and carbs just a little if it makes you feel better?
Having diabetes run in your family by no means means you'll develop it as well, so you might be worrying over nothing.
 
I find at the beginning of making changes sometimes it's just will power that needs to kick in until it becomes a way of life and that's just what you do. It does become easier to some extent. Mistakes will be made, but that doesn't mean you toss in the towel.
Perhaps writing down why you want to make changes will help solidify it in your mind. Keep it handy to refer to when needed.

I'm not a "small changes gradually" person. What changes are you talking about, and why?
