Has anyone had to fill in something like this before? It was explained to me by my DSN but I still don’t understand it. IMG_4833.jpeg

From my memory some of it looks vaguely similar to the one I had to do when I went on my carb counting course (in 2019) - I had to do a diary like that for 4 weeks in total
Though you seem to have two entries for 8am on day 1 (note: I'm not sure what all the abbreviations are - I presume you've been told what they are though)

Its just to track what you've been eating (carb-wise) and your bloodsugar before each meal and other times if you do it as well for a good number of days so they can guesstimate your carb ratios for different times of the day, and work out if your basal needs changing

My DSN filled that in just as an example.

CP = Carbohydrate Portion
BG = Blood Glucose
QA = Quick Acting and finally
BI = Basal Insulin

That’s the only information my brain managed to take in yesterday!

Ah, fair enough :)
And ty for the clarification about QA and BI.

But yeah my 'diary' I had to fill in was similar-ish from what I vaguely remember.
Just add an entry whenever you have some carbs, do a blood test, or take insulin etc :)
Probably worth adding comments if you do any exercise as well (with the time)

Personally I dont understand why carb portions are used instead of actual carbs - but nevermind :) - I'm guessing thats how your course is doing it
