Pre-diabetic, losing weight but can’t lower BG.

Hello all, I am pre/diabetic my morning test is usually between 6.2 - 6.7 before breakfast which is normally coffee (no sugar) 2 Danish toast slices and german slice sausage. After about 2-3 hours I start to feel really sleepy, didn't know why. I read normally when you are T2D. I also read exercise helps. Anyway I take my BG at 3pm and it would normally read 5.3 -6.1. What is happening. I'm going to try exercising after meals (which i only have breakdfast and evening meals *pre 8pm. I'm getting peeved now cause I've lost 8% of my body weight my HBa1C is 46 and still can't get my bg lower. When I go under 70kg I start to feel dizzy but my ideal weight should be 64Kg. So now the doctor is saying leave my weight alone now!!! What shall I do. My BP is 108/74
 
Hi and welcome. I think checking blood glucose levels is essential if you want to control your condition, but it pays to do it in a structured way. Testing at set times doesn't tell you all that much - BG varies a lot throughout the day, bioth in response to food and because your liver is continually adding glucose to your system as and when it thinks you need it. Livers can get used to higher than ideal glucose levels being "the usual" and will attempt to keep them high.

I found that using the tests to establish the foods that have the biggest impact on your glucose levels pays off. Carbs will always raise glucose levels initially, but what you really need to know is how well and how quickly your body deals with them. That's why the recommended pattern is to test immediately before eating, to establish a baseline, and then two hours later. This is not testing to see how high you go - the high point will probably be around 45 minutes after eating.

Instead this way of testing shows you how well (or otherwise) your insulin response dealt with the carbs in what you ate. Carbs are digested to glucose and ideally your insulin should get you back at, or close to, the baseline reading within two hours. If not, there were too many carbs in what you ate for your system to handle at present, and that implies that the surplus glucose stays in your bloodstream at an elevated level for longer than it should.

So if (for example) you take a reading before breakfast, and then another two hours later, the +2hr level should be within 2 mmol/litre of the first (and under 8.0 mmol/litre). If it's outside that range, your insulin isn't coping with the carbs in the toast and the best course of action, if you want to reduce your BG level, is to reduce the quantity of carb in what you ate.

Does that make sense?

My guess is your sleepiness is caused by elevated BG levels falling - I used to get this.
 
This is a graph from a Constant Glucose Monitor worn by a non-diabetic person, and it shows very clearly how it's normal for blood glucose levels vary across the day - you can easily spot where the person probably ate something.

Building and maintaining muscle would really help. Impressive that you have been able to lose a substantial amount of weight.

Downside would be how much of that was muscle? Muscle is where we store most of our glucose. Keeping as much even adding to it can be really beneficial.

Don't get to annoyed with yourself. It's about just finding what works for you and what's manageable.

Hi there @potatohead , have you thought of using a Continuous Glucose Monitor (CGM) ? Apologies if you do already, but if you don’t it may be useful as these monitors show you what your blood sugars are doing at any given moment and records them on a continuous graph, very useful. My personal view is exercise is mainly beneficial , depending on health. I use exercise to help control my own blood sugars. It’s also important to make sure your diet is sufficient to maintain your body weight, but not too heavy in carbs to push up your blood sugars. It’s about choosing the right foods for your body.
I’m a person who can lose weight very easily, I have always struggled to keep weight on, it was mainly because I was a competitive runner for many years and running takes energy and I could not eat enough for the amount of training I undertook. So exercise can be a double edged sword, certainly increasing muscle tone will help in conjunction with the right foods. My suggestion would be to look at using a CGM to find out what foods your body can handle without bumping up your blood sugars , but balancing the type of foods that maintain weight.
