Moving home....

Sooo I have finally got my own place....and well I was just wondering would I need to change who my Diabetes care is with ie my nurses etc....as well I don't really want to...I might moan about them but I don't want to change them and well it isn't really that far....I know I will probably have to change my GP as much as I do not want to :(
 
Guess it depends on the distance you've moved and if you're now out there area. Think you're going to have to pick up the phone and ask them.

Sounds as if you are getting apprehensive. A big step getting your own place, but I am sure you will be fine.

The only way to put a stop to this particular worry is to do as nuthead says and ask current health team.

If you find you cannot stay with them, try not to worry. You may have some choice who sees you so find out and remember change can be good.

Easiest way to find if you can stay with the same surgery is to look at your surgery website and see what area they cover. Also give your DSN a ring and see if you are still under them. Good luck with your move. :)

You will still be under the care of your hospital clinic if you are seen there for your diabetes, however its worth checking to see if you can still be a patient at your gp surgery. Hope your settled and happy in your new home Nicola:)

Normally you can stay with hospitals if you request to remain with them..

Are you with a community nurse though for your pump or at a hospital?

As of October certain gp's signed up to take patients out of there area's. Gp's had options to sign up or not... Only your gp can tell you that. The only thing is that it makes emergency call outs from a GP awkward and currently not sure how this is being managed.

Well done on your new home. Initially, don't go spending out on things... Make sure you always have enough money to pay your rent and utilities. There's ways and means to get furniture etc....food even.looking at reduced items first and getting a chicken in a sliw cooker can last days and can be made into cheap varied meals.

With the slow cooker, a chicken becomes, for me, Sunday roast, cold cuts for lunch, a curry, and lashings of soup:cool: (not all at once, I freeze portions!)
Also the less tender cuts benefit from a long slow cooking and end up being "cuttable with a spoon" e.g. shin beef!:)

Ahhh, I remember my first place. I furnished it in an eclectic style I referred to as "Early American Poverty". :D I loved that place!

Well my Diabetes Care is at a hospital and on the letters it always says North East London....whether that means Canning Town I do not know....I know calling my DSN is best thing to do....but can I ever get hold of her?? Ofcourse not.....like before christmas I was having problems with my set...I spent a whole day trying to get through to them to be told they was on holiday for 2 weeks...maybe thats my call to change?? But what would happen about my pump?? Would it be taken away from me????


PS Fairy Godmother indeed....don't think I would be able to do this without you!! :) You truly are an amazing person!! :)xxx
 
No, your pump stays with you.....

You either stay with your current gp and ask to be changed to a different hospital (research who has the best pp climic before asking your gp to change you to thst hospital. The hospitals then just change name as duch but your funding should continue with the same CCG if its the same CCG where you are moving.

Or you change gp in which case go on to nhs choices and type in your postcode with gp surgeries... That will then give you a list of all the surgeries available... Again look at yhe reviews of the practices on the nhs choices website.

You can also look up whether hospitals and gp's have been CQC checked and this will give you a good idea for gp practices (only if they've been reviewed since November).. This is when CQC really changed their inspections....

What ever happens you will not lose your pump and your nurses / consultants will do necessary paperwork.

1) you do not need to change hospital.. Unless you want to

2) find out if your gp is under your postcode by going on to nhs choices/gps and type in your postcode

3) if you are out of gp area, and you want to stay with your gp find out if they accept out of area patients(new in October)

4) if you have to change..change gp first, look up reviews under nhs choices and CQC inspections. Choose the gp you want to register with and take in papeework to peove you live at new address...

HOWEVER.... Order enough supplies of pump stuff for 3 months..... This can be sent anywhere you like ie work or home so long as you give them your account number and the address you wish supplies to go to.

You will be ok....you would have got your home without my little bit of background support and help....

Just keep impulse purchases to a minimumand for this year... Don't go spending fo valentines day!!!! Just cook at home and have a candlelit dinner or something.. Keep hold of every penny you can at the moment....

My GP will probably be my main issue as when my DSN sent something to my boyfriend's as a one off they put that down as my address which obviously my GP found out well my Nurses did and then they was like you can't stay with us blar de blar but I like my GP she adores me haha! She always does the best for me and has known me since I was about 2 (which she states to everyone she sees when I am with her lol) But like all said it will be for the best probably as that is life!!
 
As of October though the NHS GP's Could opt in or opt out of taking patients from outside the area....

So effectively allowing patients to register nearer their work if they needed to and wanted to...

It'll be worthwhile just checking out whether your GP practice opted in to take out of area patients or opted out. If they opted in, it would be just a case of changing your address with them....
