Confused by recent appt/advice

Hello,
I had my first face to face appt last week in diabetic clinic in our hospital (18mths since diagnosis of ‘slow onset type 1’ - last autumn was a phone call appt with the consultant, 1st one).
It was a different consultant. She told me many things , inc that I’m LADA, and all my questions went out the window as I was trying to understand what I should be doing from what she said!
I’m struggling to fully understand and remember exactly what she said about a blood glucose reading/numbers being affected from the meal before or the day before??
And that I can adjust my insulin accordingly and not to rely on constantly looking at my numbers. And not to keep eating (I was leaving the cgm app open on my phone so I can see how my levels are and then eating when going low/to prevent going too low, and my cholesterol is high).
So since the appt I’ve been better at not checking the app constantly and not eating in between meals so I feel I’ve managed my meals and levels better. Not altered insulin too much. However this morning upon checking the app it looks like I’ve been high all night - up to 15mmol and just coming down to 11mmol. My dinner yesterday evening was pretty low carb followed by an oat based dessert which was small anyway. And I had my usual 7units ( I am on low doses of insulin probably compared to most people).
So because I can’t remember exactly or fully understand what she said about the day before or the meal before is what affects the levels, I was hoping someone might shed some light on which meal affects BG levels at which point of the day or which day, especially now as last night I’ve been so high all night.
Apologies if not making sense or if no one can get what I’m saying. I am waiting for consultant to phone back with blood results at some point so intend to ask again anyway but just wondered in the meantime.

Does a meal/insulin dose affect or show up in BG levels the following day?

if the meal raises your sugar levels then yes it can or at least it has with me (T2)
cant advise re adjusting your dosing levels I have no knowledge of that

tresbia is long acting insulin. designed 24+ hours slow release. that is designed to keep glucose levels stable (flat as possible) whilst fasting. Any changes to tresbia may take a couple of days to show.

fiasp is very quick acting acting and designed to deal with your sugar levels for mealtimes

What i found handy was write a list of questions leaving a space beween, take pen to appt alongside extra blank piece of paper to write any notes down. so much easier than trying to remember everything. You can ask dsn to print off info or they may have an email address. You should hopefully have telephone number for them. can't offer dosage advice.

insulin requirements may change you'd need to speak to medical team for advice on that front. If running high there are correctional doses you can do, if haven't received medical advice from professional for that suggest you call them and ask whilst on phone ask if can go on course such as dafne or when are you able to speak to a dietician to help work out ratio's etc.

took me several weeks before blood glucose levels stablised after inititally started insulin, and needs changed a LOT since then.
 
Hi. Be aware that Carbs will affect your BS levels far more than Fats or Proteins. If you make a big change to the Carbs you may need make a change to your Fiasp injections so seek advice from the GP etc. The body's BS will react to Carbs within 1 hour or so. It may take longer if there is also Fat with the food. Ask the medical team for advice on altering your Fiasp based on the Carbs you are eating.

As you are relatively newly diagnosed have you been sent on any carb counting and insulin adjusting courses? A lot of people here speak well of DAFNE but if that isn't available there is an online course which may be helpful.


I suspect that as you are newly diagnosed you may find that your insulin needs increase over the next year or so. Hopefully your team have discussed how to alter doses with you?

In any case, the way I was taught it is to adjust my basal dose so that levels stay steady overnight, then adjust bolus ratios so that they cover your meals.

Fiasp takes up to 5 hours to get out of your system, so if you compare your bg 5 hours after your evening dose with the value in the morning, you should be able to see what overnight effect your tresiba is having. (However, that assumes your evening food is also out of your system by then, which it might not be if you have a bedtime snack and/or eat something like pizza.)
 
Hi Em - I'm also on Tresiba (at my request - as indeed it's a lot more stable than previous Abasaglar, which I found for my condition was too reactive). However I am also finding response times very long sometimes as so I can concur with the comments from others to your post as well as your observations). I've suspect a decline in my insulin production since last A1C1 test, as I've had to increase my Tresiba from 9 to 11 U.. but I've also been finding that I often need to make 'micro' adjustments (2U) depending on what I eat recently to stay in range... but the effect is really slow so I may also look at Fiasp.
