Hi, I'm new to the forum and am seeking advice on levemir doses. I take 16units in the morning and 12 units at night. Im finding i always have high blood sugar readings in the morning, up to 14mmol. I don't think I'm going low overnight as i usually wake up if i have a hypo. I've tried having different sugar levels when going to bed but doesn't seem to change things. Don't now if i should be increasing or decreasing my levemir dose! Does anyone have any ideas?

I split levemir, to combat my high morning levels I take more in the evening than in the morning (10 in the morning and 14 in the evening) as I think my morning dose has more or less run out by the following day. I rarely go hypo overnight unless there are other factors involved (low before bed, alcohol, wrong bolus for dinner etc) but on a "normal" day I'm fine.

Are your levels during the day ok? If so maybe increasing your bedtime dose would help or altering the timings of the dose. Speak to your DSN

Thanks for your reply. I was told that generally you should take less levemir at night as your not eating overnight but clearly my dosing is not effective! My sugar levels in the day are generally fine and I have reasonably good control. I used to take lantus so levemir is quite new for me. I may try gradually increasing my evening dose and if that helps. Thanks again.

I would be inclined to increase the night dose by a unit or two and check your bg around 2 and 4am just to see how your bg is behaving, if you like try checking your bg tonight in the early hours of the morning before making any adjustments, if it's high then increase the dose.

Good advice from Noblehead. I was also going to say the same thing.

If only people would get up in the night and test their bg levels and eat some carb if necessary, so many would save themselves from experiencing a serious hypo. I always make sure that I drink a cup of coffee or tea before going to bed just to ensure that I do get up in the night.

I got up in the night before, and do on the odd occasion to check that I'm still on track. But u definitely do t do it every night. Broken sleep like that makes me sluggish the next day, I don't go back to sleep easily after getting out of bed!

My understanding is (and I'm sure someone will correct me if I'm wrong) is that your levemir dose has nothing to do with eating, for example if you took your levemir as normal and then didn't eat all day your levels should remain steady and stable hence it is the basal. A couple of sleepless nights basal testing and getting your dose right could save a lot of trouble in the long run

Yes you are correct that background insulins dont really affect the carb that we eat where we use bolus but I'm sure there are many like myself that find that their background basal insulin often needs to be adjusted because of bg changes and nightime changes to bg levels do happen. Sometimes I need to make adjustments but if I didnt bother to get up and test I'd either find myself waking up to really high bg levels or low bg.

Hi I am on split dose of levemir , I take 10 n the morning and 22 at night and that helps with my bloods in the morning. I am doing a study for diabetes and am in close contact with the specialist nurses and they really help with my doses.

I take split dose levimir 20/20 my bg was goin up in a morning so I performed a 3am test as recommended by a dietitian and Dan while I was on the DAFNE course my bloods were up at 3am so I went from 19u to 20u I was taught to go up 1u and down 3u when needed, hope this helps

Having done night time blood glucose testing I've increased my night time dose of levemir. Ive also tested through the night since the dose adjustment and i seem to be on the right track. Thanks for all the info, really helpful.
