A concern? Please help

After a chat with my diabetic nurse today, I got told not to have a insulin injection of 3mmol with my evening meal (around 5pm) because I have football at 7.30pm. She said before you leave for football take your sugar level and I did and scored 17.4mmol which is high and ketones came up aswell. I am concerned alot because of how high the score is, i know atm its trial and error but does this show I need the injection before my meal??

I had half a tin of spaghetti on wholemeal toast with tuna and low fat/sugar cheese.
Also a cuppa tea and 2 digestive biscuits. Around 5pm I had this meal.

As i scored 17.4 I've just been for a run before football to try and lower my sugar level score so it isnt to high.

Hope you can help and settle my concern.

hiya not sure about what u should be doing insulin wise but i know my DSN says dont exercise if u have ketones and that u need extra insulin to get rid of the ketones so be careful exercising with high bg and ketones

Hello Mathew

Most bg meters will flash a warning to you to test for keytones if a high bg level is recorded such as your one. If doesn't necessarily mean that you have keytones though, its just a warning. Most people get their bg down by giving some insulin although if people feel fairly ill, then they should go to hospital or ring a doctor. Keytones usually give off peardrop taste in the mouth with dark urine. Usually insulin and drinking regular amounts of water gets rid of them.

I understand your dsn's concern over you doing exercise like football. She's worried that you will go hypo so that is why she has told you to leave off your bolus. I think it might be better for you next time to eat food but only give yourself half your usual bolus and then that hopefully will be ok to deal with bg lowering effect that the football may produce and keep testing. Always carry some form of glucose in your pockets when playing sport for sure.

Ams is right though, exercise might actually push your bg levels up if they are already high so be careful and keep testing every 60-90mins to see which way your bg levels are going.

I have a feeling that you are going to need to give yourself a bolus correction dose sometime this evening. I would speak to your dsn tomorrow and tell her what has happened to you today.

I'm not surprised that you hit 17.4 mmol/l. What you ate is a hefty dose of carbohydrates without anything to cover them. Did your DSN tell you to check the pre-meal reading first ? You need to know exactly what sort of bolus you would need for the meal. Common sense really. Let her know what happened as iHs said.
 
Before the evening meal I scored 2.8 which is hypo and i didn't inject because I had football later on in the evening and got told not to and before football then i scored 17.4 which has been stated. Played 2 games of football, came home and scored 9.1mmol sooooo...whats happened?

Do know that with heavy exercise your liver dumps extra sugar into you to give you energy.
so going for the run might not have helped you at all.

I'm type 2 so don't know about insulin but thought the info on the liver might help.
