Advice on Transition from Pump to MDI

Hello, I hope my message finds you well.

I was wondering if I could ask for your advice regarding the following matter.

I have decided to take a short break from pump therapy (Omnipod Dash & Dexcom G7) and transition to a multiple daily injection (MDI) regimen. I would be most grateful for your suggestions as I navigate this transition.

I have been suggested Lantus or Abasaglar or Tresiba as my basal insulins.

By way of background, when I was first diagnosed, I was prescribed Lantus as my basal insulin. Unfortunately, my experience was not favourable, as I found insulin glargine to be somewhat erratic, with episodes of sudden hypoglycaemia, as well as localised reactions including redness, stinging, burning, and swelling at the injection sites. This led to my subsequent switch to Tresiba.

My experience with insulin degludec (Tresiba) was very positive overall, as its stable and flat profile provided consistent glycaemic control with minimal hypoglycaemic episodes, even following exercise. However, I found that my insulin requirements differ between day and night, requiring a higher dose during the day and a lower dose overnight. As Tresiba cannot be split, this resulted in nocturnal hypoglycaemia despite satisfactory daytime control. My diabetes nurse suggested that this could be because the overall dosage was high.

For this reason, I was later transitioned to Levemir, which allowed for split dosing (previously 13 units at 06:00 and 8 units at 21:00, to the best of my recollection). I understand that Levemir is due to be discontinued, and therefore I would be grateful for your advice on a suitable alternative.

I have not previously used Abasaglar; however, given its similarity to Lantus, I would anticipate a comparable response and potential side effects.

I would be grateful for your views on the most suitable basal insulin (Lantus, Abasaglar, or Tresiba), as well as any guidance on:

1. Timing of the first basal dose when transitioning from pump (I am wondering whether the first dose of Tresiba should be administered while the pump is still active in order to avoid any basal insulin gap, and if so, the appropriate timing before discontinuing the pump).
2. Converting my current basal dose (currently ~10.15 units/day; TDD ~20–22 units)
3. Whether Tresiba requires 3–5 days to reach steady state
4. Morning vs evening basal administration

I would also value reading about your personal experiences with a similar transition.

I have not yet set a date for the transition, as I would like to feel fully informed and confident before proceeding. I must admit that I feel somewhat apprehensive about moving away from pump therapy, even temporarily. My intention is to return to pump use once I feel ready.
