Any one hear of Diabetes Distress Survey?

I am a program coordinator in a mental health agency and I also run a diabetes support group. I am on the internet all the time looking for new stuff and I recently found an article on this Diabetes Distress Survey which can be taken to ascertain how difficult it is for you to manage self-care, emotions, interpersonal problems involving diabetes and faith in your doctor. Has anyone bee given this test by their doctor or other care providers?

I guess I should say I am an American in America.
 
I am a Canadian in Canada and I have never heard of it either.
 
I've been a bit involved with the South London Health Innovation Network that's put this together. They have piloted the study with Guy's Hospital and King's College Hospitals and it has been rolled out to all patients at their clinics.

I am in the UK but very depressed about being diagnosed Diabetic ( type 2) I have/had PCOS ( can I still have this with no womb, ovaries or tubes? they removed due to cancer) I so angry about the PCOS which went undiagnosed for many years, in my teens in the early 80s it wasn't well known sadly. Anyway PCOS caused me to not only be hairy and look very manly but made me infertile, lead me to have womb cancer and joy of joy now Diabetes type 2. I swear this PCOS thing will kill me.

I am in the UK, type 1 for 11 years and never heard of this !
